Dear Mr. O'Reilly:
This confirms the telephone conversation at 0840 on January 27, 1983, between Mr. R. M. Poulk, Jr., and Mr. C. W. Hehl, concerning an immediately reportable event per Technical Specification 6.9.1.8c.
While Southwest Research Institute contract personnel were performing surface surveillance tests on the "H" riser of the "B" recirculation loop, leakage was detected on weld B32-recirc-12-BR-H-5.
This weld connects the riser pipe to the safe-end. The leakage was detected in the heat affected zone on the pipe side of the weld in an area where there appears to be an arc strike. Further evaluations are currently being performed to determine the failure mechanism and will be reported in the 14-day report.
