Dear Mr. O'Reilly:
This confirms a telephone conversation between Mr. R. M. Poulk, Jr. and Mr. C. W. Helh at 1415 on January 18, 19P3, concerning an immediately reportable event per Technical Specificat_on 6.9.8c.
While performing testing required by IE Bulletin 82-03, it was determined that weld B32-Recirc-28-A-15 had two crack indications. These indications were in the heat-affected zone on the pipe side on the weld which connects a right angle elbow to a straight section of pipe (see enclosure). This pipe is located downstream of the A-loop discharge valve F031A. Computer analysis of these indications indicate that one is approximately 11 percent of wall thickness and the other is 5 percent of wall thickness. TS indications appear to be one-half to three-fourths inches in length on che circumference of the inside diameter of the pipe. The pipe is O pe 304 stainless steel with a wall thickness of 1.32 inches. Further investigation is being conducted on these indications at this time.
