liOFTH ANNA POWER STATION UNITS 1 AND 2 SUPPLEMENT TO PENETRATION ElBE BARRIERS SPECIAL REPORT Our letter dated August 5,1993 (Serial No.93-481) provided you with a Special Report identifying several blockout and sleeve type penetrati~ n fire barriers that were o

determined to be nonfunctional. This supplement to that report is being submitted to identify two additional fire barriers, representing a third type of fire barrier, that were determined to be nonfunctional. Subsequently, repairs were completed and the fire barriers were restored to functional status.

1 On March 29,1995, during the performance of the detailed surveillance inspections of

the fire protection system fire barriers, the gap seals between the top of a 12 inch concrete block wall and their associated steel "l" beams above the block wall were discovered filled with everlastic column boxboard. This condition was noted above the wall between the Unit 2 Cable Spreading Room and the Turbine Building, and the wall between the Unit 2 Normal Switchgear Room to the Unit 2 Mechanical Equipment Room. A fire watch was established until such time that the fire barriers were repaired using silicone foam and restored to functional status. This supplement is also being submitted because these deficiencies are believed to be the result of inadequate initial installation.

An engineering review was initiated whereby several fire barriers that have the potential for a similar configuration were identified. These barriers will be inspected on an accelerated frequency, and appropriate compensatory and corrective actions will be implemented, as required, to restore them to a functional status. Any similar deficiencies will be documented by station deviation reports. This approach is similar to that applied to previously identified discrepancies with the blockout fire barriers.
