The NRC was notified of a potentially reportable deficiency in the referenced telephone conversation. At that time, it was estimated that a determination of reportability would be made within thirty (30) days.

, Due to the extensive investigation and evaluation required, an Interim Report is attached.. It is now expected that this information will be finalized by February 9,1984, at which time a complete report will be submitted.

E INTERIM REPORT - DER 83-77 POTENTIAL REPORTABLE DEFICIENCY ARIZONA PUBLIC SERVICE COMPANY (APS)

PVNGS UNITS 2 & 3 I.

POTENTIAL PROBLEM Subsequent to pour 2-C-123, NCR CC-2773 identified Containment Dome surface as being rough and several areas had not been trowe11ed. Weather conditions during the pour and finish were rain and wind. NCR disposition was use-as-iE, based on concrete showing no signs of unconsolidation.

Reference HCR CC-4338-identified honeycombed concrete in five (5) blockouts.

Interim disposition No. 2 to NCR CC-4338 identified additional honey-combed concrete, resulting in no further chipping on the containment dome, pending engineering review of the condition.

Reference NCR CC-4418 A defective area exists in the concrete of the exterior face of the Unit 3 Containment Dome.

In chipping the concrete to determine the extent of the defect the sheathing for tendon H21-050 was damaged.

The tendon was not damaged, however it has been tensioned.

II.

APPROACH TO AND STATUS OF PROPOSED RESOLUTION Bechtel engineering is currently taking soundings of Unit 2 and 3

-containment domes to determine the extent of the honeycomb areas.

Based on the results of the testing a corrective action plan will be developed.

III. PROJECTFD COMPLETION OF CORRECTIVE ACTION AND SUBMITTAL OF THE FINAL REPORT Evaluation of this condition and submittal of the Final Report is forecast to be completed by February 9,1984.
