Dear Gir:

The NRC was notified of a potentially reportable deficiency in Reference A, and an Interim Report was transmitted by Reference B.

At thet time, it was estimated that a Final Report would be available by November 30, 1983.

Due to the extensive investigation and evaluation required, a revised Interim Report is attached.

It is now expected that this information will be finalized by February 15, 1984, at which time a complete report will be submitted.

PVNGS UNIT 3 1.

POTENTIAL PROBLEM Construction installation of Type N embed plates in Containment 3 failed to meet specification and WPP/QCI requirements.

A damaged beam in NW Qual at the 100' 6" elevation was being removed per NCR CC-4043 disposition, when it was discovered that two (2) Type N insert plates were installed without the four (4) 1" X 9" lg. suchor bolts.

Concrete post placement inspection of insert plates for 3C081 has not yet been performed.

In addition the following listed NCRs summarize the embed plant / anchor bolt problems by specific NCR number.

II.

APPROACH TO AND STATUS OF PROPOSED RESOLUTION An Engineering walkdown team was formed to investigate embedded plates listed in the NCRs to determine the structural adequacy of the cited embedded plates. The results to date can be summarized as follows:

1.

The anchor bolts with visible insufficient thread engagement were plug welded in accordance with Section 7.4 of Spm ification No. 13-CM-3 08.

2.

Embedded plates with identified missing bolts (1 or 2 missing

/ i bolts per plate) were evaluated individually, and found to be structurally acceptable as the "as built" plate design capacities are higher than the applied loads. However, DCNs have been issued for all the plates with missing bolts to prevent additional attachments or loads.

3.

The two defective type N plates in the Unit 3 Containiment Building were replaced by new plates with Maxi-bolts (documented in NCR CC-4201).

One embedded plate on the Turbine Pedestal with all bolts missing was removed by DCN and deleted' from Unit 3 engineering

~ design drawing.

5.. Embedded plates with bolts covered by attachments were evaluated individually and all were found to be structurally acceptable except for 3 evbedded plates in the Containment Building which still are under investigation.

III. '- PROJECTED COMPLETION OF CORRECTIVE ACTION AND SUBMITTAL OF THE FINAL REPORT Evaluation of this condition and submittal of the Final Report is forecast: to be completed by February 15, 1984.
