Dear Mr. Keppler:

This letter will serve as an interim report pursuant to 10CFR50.55(e) on the po-tential significant deficiency concerning certain attachrmnt welds to the ASME Code containment pressure vessel which now requires evaluation for Code empli-ance relative to exmption frcm post-weld heat treatment requiremnts. This matter was first reported by Mr. C. Shuster of The Cleveland Electric Illuminating Cmpany to Mr. P. Pelke of your office on November 10, 1983.

This interim report contains a description of the deficiency and planned correc-tive action.

Description of Deficiency During in process ANI hold point inspection, it was noted that certain attachment plate to Containment vessel shell fillet welds exceeded the maximum throat thick-ness (3/4") for exmption from post-weld heat treatment (PhWP). A detailed review by Newport News Industrial Corp. of Ohio (NNICO) QA/QC of all attachment plates in excess of 1-1/2" thickness had identified fourteen plates with fillet welds in this category, seven in Unit 1 and seven in Unit 2.

Further investigation has revealed an inconsistent interpretation of ASbE Code requirments for exmptions fran PhWP as clarified by ASFE Code Interpretation

  1. III-81-49 dated May 12, 1981. Newport News Industrial Corporation of Ohio and Newport News Industrial of Virginia incorrectly defined the rrethod for determining the governing base material thickness for ASbE welding as the thinner of the parts being joined.

'Ihis misinterpretation resulted in a number of attachment welds to the containment vessel receiving neither PWEff nor adequate preheat to exenpt then frcm PWEff per the ASME Code.

Corrective Action NNICO.is currently in the process of verifying the as-built sizes of all fillet welds in material greater than 1-1/2" in thickness and also verifying the preheat used in all welding of materials in excess of 1-1/2" in thickness. Nonconfornance Report 17-439 has been subnitted documenting conditions found to date in Unit I.

Nonconformance Report 17-449 has been written to track future evaluation for Unit II as information becanes available.

NNICO is revising their procedures to incorporate the correct ASME Code interpre-tation relating to exemptions fran PWIrf. All areas identified by NNICO as non-conforming will be brought into canplete canpliance with the ASME Code either by performing the required PWEff or by physical repair.

We are presently planning to subnit our next report on this subject by May ll, 1984. Please call if you have any questions.
