10 CFR Part 20.1003 depnes a radiation area as an area, accessible to individuals, in which radiation letwls could result in an individual rewiving a dose equivalent in excess of 0.005 rein in one hour at 30 centimeters pons the radiation source or frorn any surface that the radiation Jenetrales.

Reason for Violation The'594 foot' elevation is posted by individual areas due to the small number of radiation areas rather than posting the entire elevation. All j
other elevations are posted as Radiation Areas due to the number or configuration of areas meeting the requirement for posting.
j Surveys and posted areas are documented in routine surveys performed by Radiation Protection. The area in question was surveyed on February 18,1995, and found not to meet the requirement for posting. During recent refueling outage (1EOCd), unexpected crudburst conditions elevated plant dose rates significantly higher than normal due to activity j
carried in primary coolant systems. Upon entering the refueling mode, this activity transferred to refueling systems. With the exception of the 594 foot elevation, these systems were properly monitored and posted as
.l noted in paragraph 5.d. of the inspection report details.
The root cause for this violation is attributed to inadequate procedure for surveying plant areas under changing conditions.

Corrective Actions Taken and Results Achieved The 594 foot elevation was immediately surveyed by Radiation Protection. The inspector identified area and one licensee identified area (Spent Fuel Cooling system in the same vicinity) were psted to comply with regulations.
In addition, other changes in plant configuration that could potentially affect radiation levels were evaluated for possible revision to radiation protection ro itine survey procedures. This evaiuation determined that no changes are required. Step 5.1.2.3, in Radiation Protection Manual Directive III-2, states that if any area shows unexpected changes in radiation levels since the last routine survey was performed, to attempt to determine the cause and notify Radiation Protection supervision.

Corrective Actions to be Taken to Avoid Future Violations E
. Procedure HP/0/B/1000/43, Radiation Protection Control of Work in Upper Containment, shall be revised to require surveys on the 594 foot elevation in and around the area of the spent fuel pool cooling piping upon commencement of refuel activities. This revision shall be complete -
