NOTES

1) Valves were tested individually. The " worst case" leakage from containment is given by the minimum value of the leakages for the two valves in series.
2) The two valves were tested together. The LLRT result is the total leakage through both valves.

Since the volume tested is between isolation valves, the " worst case" leakage would be when both valves leak equally (i.e., one-half the LLRT result).

3) 'Only one of the two isolation valves was repaired. Therefore, the "as left" LLRT result also represents a " worst case" leakage for the unrepaired isolation valve.
4) The MSL drain valve, 1-220-1, caused the LLRT failure in 1979 and 1982. The plug in the globe type valve was replaced with an "over sized" component in 1982.
5) Corrective action required. Note that the " worst case" leakage through this flow path from the containment was not a problem in 1979 and 1980, but was unsatisfactory in 1982. For this reason, preventative maintenance should be the most important feature of the corrective action plan.
6) Corrective action required. This isolation consists of an untested stop check valve and a single, tested check valve. This LLRT of a single check valve on a steam line warrants an engineering change to increase reliability. Note that the LLRT results in 1979 and 1980 gave no warning of the impending failure in 1982.
7) The LLRT results in 1979 and 1982 required repairs to the 1-1601-55 valve. Because of multiple isolation features, this single valve failure would not have resulted in excessive containment leakage.

If failures of this valve continue, future corrective action will be required.

8) This valve was replaced in 1982 with a different valve design in an attempt to improve reliability.

r CORRECTIVE ACTION PLAN BASED ON FALL, 1982 TYPE B AND C TESTING ON UNIT ONE

1) FEEDWAL.R INLET VALVES a) During the upcoming refuel outage, all four isolation valves will be dis-assembled to install additional hold down clamps on the valve seat rings.

b) During the above modification work, all pin bearing surfaces, seats, and seals will be inspected and repairs made, as necessary.

c) A preventative maintenance program on these isolation valves will be initiated.

(1) One (1) isolation valve will be overhauled in each feedwater line (with new seals, seats repaired as necessary, and all moveable parts repaired as necessary) each refueling outage regardless of LLRT result.

(2) In no case will FW inlet valves, regardless of LLRT result, go more than two complete cycles without the above repairs.

2) DRYWELL HEAD SEAL

)

a) Initiate a preventative maintenance program.

(1) Maintenance procedure written to:

(a) Prohibit re'-use of seal when drywell head is installed.

(b) Implement recommendations of NRC I.E.Bulletin 78-09, (b) Increase testing frequency.

(1) Presently, an "as found" LLRT is only performed when a Type A test is to be performed during the same refuel outage. An LLRT will be performed on the drywell head prior to its removal

("as found") every refueling outage.

(2) For cycles 8 and 9 on Unit One, the drywell head will additionally be LLRT once at approximately the midpoint in the power cycle (i.e., a test frequency of 9 months t 3 months). The duration of more-frequent testing will be extended if any failures are found during cycles 8 and 9.

3) HPCI AND RCIC TURBINE EXHAUST CHECK VALVE NOTE: These isolations are multiple isolations in that each har a check valve and a stop-check valve. The stop-check valve is only tested in conjunction with the check in its mode as a stop valve, and not as a check valve. Because the LLRT of a single check valve causes a technical failure of the "as found" Type A test, an engineering change will be pursued.

Option A: (Preferred)

(1) Add a second check valve to each flow path.

Two check valves, individually testable, will allow an added measure of redundancy and reduce the likelihood of a flow path from the containment causing an "as found" failure of the Type A test.

Option B:

(1) Replace the existing check valve with a new valve to increase reliability.

(2) Verify improved reliability by more frequent LLRT's.

Perform LLRT during cycles 8 and 9 on a 9 months 3 months frequency.

The duration of more frequent testing will be extended if subsequent failures are found.
