The label is to bear the radiation caution cymbol and the worda " Caution, Radioactive Material *', and also,erovide cufficient infornation to permit in-dividuals handling or using the containers, or working in the vicinity thereof, to take precaut.'ona to avoid or minimize exposures.

Contrary to the above, on July 15-16, 1991, the licenree iailed to labe) with a durable, cicarly visible label, two B-25 boxes containing radioactive material in cycess of 10 Crn Part 20, Appendix C limits which were ntored on the Radioactive Materia]n Control storage pad, northwest of t he Unit 1 con-tainment bu'.1 ding.

This is a Severity Level IV violation (supplement IV).

The rearon for the violation:

During the MRC Inspection, the innpectorn found a faded Rad Material Tag (s ) fastened t.. b-25 storage boxes (outnide RCZ for Had Materini Storage).

Added step 4.6.1:

Each unsecured area (open to elements) shall have sufficient yellow tage on each individual item to assure identification per RP Manuni Section 13.2.
