DOCKET NO, 50 445 AND $0-446 RESPONSE TO ALLEGATION NO, 4-91-A 006P Gentlemen:

By letter dated July 26, 1991, the NRC requested TV Electric review an allegation pertaining to the cause of a July 2,1991, event where non-radioactive condensate water was spilled in the turbine building.

The spillage occurred during preparations inr planned valve maintenance on the condensate polishing system.

Prior to the receipt of the allegation from the NRC, TV Electric assembled a special task team to review the event to determine causes and recommend corrective actions.

Following receipt of the allegation from the NRC.

TU Electric Corporate Security initiated a review of the ev nt.

Both review activities have been completed.

The special task team determined that the immediate cause of the event was the failure of a temporary mechanical restraining device used to secure a valve in the closed position.

A numidr of other factors were determined to contribute to the event including the configuration of the temporary restraining device, the design of the valve en which the temporary restraining device was installed, and the valve's response to a loss of crerating air pressure, it was further determined that while there were shortcomings in personnel performance which contributed to this event, the individuals who installed the temporary restraining device did so as part of a conscientious effort to assure that the components to be worked on during the planned maintenance activity were effectively isolated.

o The results of the review conducted by TV Electric Corporate Security sup-ported the findings of the special task team.

It is therefore concluded, that the allegation was not substantiated.
