Dear Mr. Keppler:

This letter serves as the final report on the potential significant deficiency concerning Bailey 720 Utility Station devices supplied by General Electric (GE) in various Control Room Panels and Local Panels located in the Power Generation Control Cornplex (PCCC). This item was originally identified to Mr. McGregor of your of fice on June 26, 1981, by Mr. W. Kacer of The Cleveland Electric Illuminating Company (CEI). Interim reports nn this subject were submitted July 24, 1981, and April 29, 1983.

This report contains a description of the deficiency, an an21ysis of safety implication, and the corrective action taken.

Description of Deficiency Due to irregularities in the plastic part used in the switch assembly, push-buttons in Bailey 720 Utility Station devices may stick in an open or closed position. This condition was identified to CEI by GE on May 13, 1981.

Analysis of Safety Implicap on The Bailey 720 Utility Statioi. devices are utilized for safety-related manual control functions. A defective device could cause a false operating signal to be transmitted during manual operation. This could result in the technical specifications being exceeded during a postulated plant event.

Corrective Action All Bailey 720 Utility Station devices contained in the Unit 1 and Unit 2 PCCC panels were removed and returned to Bailey for rework as required by GE Field Deviation Disposition Requests (FDDR) KL1-671, KL2-600, and KL1-2007. All the 1

Kappist November 29, 1983 station devices but the three (3) identified on FDDR KL1-2007 and relay 8832 on FDDR KL1-671 have been returned to Perry, inspected, accepted, and installed in the PGCC panels. Bailey is expected to return the remaining devices before February 1, 1984. Upon receipt, the devices will be inspected and, if acceptable, installed in the panels. - It is anticipated that all corrective action concerning this matter will be completed by March 1, 1984.
