Dear Mr. Keppler:

Thi:s is to confirm a telephone report made by our Operations Superintendent, Mr. K.R. Baker, to Mr. O. Merrill, NRC Operations Center Duty Officer, at 1018 hours0.0118 days <br />0.283 hours <br />0.00168 weeks <br />3.87349e-4 months <br /> on November 19, 1983. At this time Mr. Merrill was advised that while performing an investigation into why a high oxygen level had existed in one of the Plant's gas decay tanks it was discovered that Action Statement (b) of Technical Specification Section 3.11.2.5 was not complied with.

This Technical Specification requires that when the concentration of oxygen in the waste gas holdup systera or tank is greater than 4% by volume and greater than 4% hydrogen by volume without delay suspend all additions of waste gases to the system or tank and reduce the concentration of oxygen to less than or equal to 3% or the concentration of hydrogen to less than or equal to 4%

On November 3, 1983, while venting Unit Two's volume control tank to one of the Plant's gas decay tanks (No. 3) the concentration in the waste gas holdup tank was analyzed and found to exceed 4% for both oxygen and hydrogen, however, additions to this system were not suspended and filling c' this tank continued for a period of sixteen (16) hours.

As required by Technical Specification 6.9.1.12, a follow-up report will be submitted within fourteen (14) days.
