1 I (1) A foreign lower adjusting ring pin was found .{ in the safety valve. That is, the ring pin 4 was not a Dresser's manufactured part'and.was i not in conformance with design criteria. } Also,'the tip of the pin was worn to the point of failure. (2).The seal wirc, for the adjusting' ring pins,- had been cut and sttek' back into' the seal. The seal had been re-crimped with regular pliers. (3) The upper adjusting ring ~was found in-a different position from the' position previously set'by Dresser. 1.2 If.the pin had. failed in service, the safety valve would not i have performed its safety function. - 2.1 Since adjusting ring pins are essential to the safety function, we are concerned about the other safety valves at Arkansas Nuclear One (ANO).
If safety related parts are + used that'do not meet design criteria, then we question the ability of the valves to execute their safety function. 2.2 Because it is a safety concern, we have opened an investigation to determine if this nonconformance~is reportable as required by 10 CFR Part~21. ANO must inform-us of their corrective action regarding this nonconformance so we can act appropriately. This reporting must be within the sixty day time limit established by 10 CFR Part 21 starting from 03-04-95. 2.2 This is not the first occurrence where foreign material was found in a Dresser's safety valve at ANO. A foreign stop pin was found in the top spring washer of a 31759 safety valve during a 1992 repair. See references 3.10 and 3.11. However, in this case, it was not a safety concern. h 4 l l

f 3 DRESSER INDUSTRIES Report SV-404 Industrial Valve Operations Page 4 of 6 Revision 0- - 4.0-Sequence of Events 4.1 In October - November 1990, safety valve serial number BM-02578 was refurbished at Wyle Laboratories.- The ' Inspection Plan and Report EG-50 (references 3.6 and 3.7)' documents that the lower adjusting ring pin met the acceptance criteria. The adjusting rings were adjusted to generic setting of -8 notches (lower ring) and +160 (upper ring) using ring position gages. This' ring setting provides. a 3 to 9% blowdown range. The ring positions were verified by Dresser after function steam test by Wyle Laboratories using no-go gages. The ring pins were then wired and sealed by Dresser. 4.2 In February 1995, valve serial number BM-02978'was returned to Wyle for refurbishment. The nonconformances noted in the summary and in Nonconformance 95-07 (Appendix 1) were discovered during the disassembly and inspection. The Inspection Plan and Report EG-50 (reference 3.4) documents that the lower adjusting ring pin does not meet the acceptance criteria. Also the upper adjusting ring had been moved from its original 1990 setting. o e
y DRESSER INDUSTRIES Report SV-404 . Industrial Valve Operations Page 5 of 6. Revision 0 5.0 Safety Concerns 5.1 The operating principles of safety valves are defined in Appendix 3. Figure 3 illustrates the adjusting rings and adjusting ring pins. 5.2 The Adjusting Ring Pin 1 The outside diameter of the adjusting rings has slots. The tip of the adjusting ring pin fits loosely into a' slot to prevent rotation. The internal basic threa'd size of the adjusting rings is larger than the corresponding external thread on the nozzle. This allows for thermal expansion of the nozzle. In the cold condition, the' adjusting rings' easily move back and forth as limited by the adjusting ring pins.- During startup, system vibrations cause the rings to twist back and forth. This wears the tip of the ring pins and may cause fatigue failure. To overcome wear and fatigue, we manufacture the pins from martensitic stainless steel (ASTM A565 Grade 616). We then heat treat the pins to.obtain a hardness of Rockwell 32 and shot peen the surface..Also, we use a radius at the junction where the tamper meets the threads to avoid stress concentration. The lower adjusting ring pin found in the safety valve did not meet the design parameters of hardness, strength, and toughness. Because the pin was of soft material,.the tip of the pin was worn to the point of failure. See Appendix 1 and 2 regarding the "as found" conditions. If the pin fails, the lower adjusting ring will most'likely rotate downward until it stops on the nozzle threads. At this low position, the safety valve will not attain rated lift and capacity. Thus, the safety valve will not execute its safety function. 4

We use special pliers for twisting'the wire'and for. crimping l + the seal. The seal pliers leave the Dresser logo across the: face the seal. The seal had knurled marks half away_across it face.on both sides. A non-Dresser person had recrimped the seal using standard pliers. Also, the wire had been previously cut. We have a concern because the seal and wire had been reused. This appears to be a deliberate attempt to hide' tampering. 5.3 The Upper Adjusting Ring Position Page 3 of Appendix 1 defines the "as found conditions". When the upper adjusting ring is set with the. gage, its-bottom surface is approximately one eighth of an inch above. the bottom surface of the disc holder. The bottom surface of the upper adjusting ring was found.approximately one sixteenth of an inch below the disc holder. At this position, the blowdown of the safety _ valve may be upto 15% of the' set pressure. This longer blowdown may be_ greater than the acceptable value established by the overpressure protection report. 9

L FA-223 Page 2 of 4 A lower adjusting ring pin was sent 1;. by Entergy Operations, Inc. from a 3707R at Arkansas Nuclear One, Unit 1, in Russellville, Arkansas. Figure 1 illustrates the condition of the pin. The pin is made of 304 stainless steel with a hardness of less than 22 Rc (Figure 2). A Dresser IVO manufactured pin is made of ASTM AS65 Grade 616, with a hardness of 36-42Rc. Figure 3 illustrates the surface condition of the pin, and Figure 4111ustrates the surface condition of a Dresser IVO manufactured ring pin. In addition to being made from the wrong material, the surface exhibits a high degree of roughness and deep tool marks. The Dresser 1VO manufactured pin shows the correct peened surface. This peening creates a very high magnitude of compressive prestress which would have to be overcome by an applied load before fatigue cracking, a most common o failure mechanism for ring pins, could occur. The result is an increased endurance limit / lifetime for the pin. i

Valve Serial Number BM 2978 was as-received tested for set pressure and seat leakage with steam as the test medium. The valve was refurbished and retested for steam set pressure and seat leakage. The testing was performed in accordance with Wyle Laboratories' Test Procedure 1062, Revision O and Entergy Operations Contract No. NAAM1071-NACM0025. 6.2 Valve Refurbishment Following the February 28,1995, As Received Steam Test, Valve S/N BM 2978 was fully disassembled, cleaned, and inspected. The compression screw and lower ring pin were replaced. The new parts were supplied by Entergy Operations. The seating surfaces were lapped and polished and the valve was reassembled and released for steam set pressure and seat leakage testing. SUMMARY
Valve Serial Number BM-2978 was disassembled, refurbished and tested for set pressure and seat leakage. The valve was again refurbished and tested for set pressure and seat leakage. Saturated steam was used as the test medium. The testing was performed in accordance with Wyle Laboratories' Test Procedure No.1009 Revision D, dated July 23, 1985, and Entergy Operations, Inc., Task No. 029-A.
