The routine rejacking was completed and the load locked off.

Pier Ke 10: The initial load transfer was complete and the routine rejacking sequence was started.

Ke 11 - Kc 10 Drift: The reinforcing steel installation was complete and the embed installation is nearing completion.

I J. O. No. 1435B Midland Plent Units 1&2 Independent Assessment of Underpinning Pier E14:

Concrete was placed in the lower portion of the shaft.

Kc10 - Kc9 Drift :

The drift has been advanced approximately 10 feet.

W 8 Crillage:

The routine rejacking was completed and the load locked off.

Pier Ke 3 :

The removal of the previously placed defective grout patch on the turbine building slab was completed and the void was regrouted.

Kc2 - Ke 3 Drift:

The drift is nearing completion with only a few cubic yards of fill concrete left for removal.

Pier W 14 : Concrete was placed to a level above the embedments near the top of the pier.

UAT:

Pressure grouting of the soils adjacent to the east and west UAT's was completed.

Cutting of the opening in the steel wall of the UAT's, to allow access for drifting to CT 1/12, was started.

SWPS:

The installation of the exterior wells and soldier piles is nearly complete. Work was temporarily halted because of a stop work order on all drilling and pile installation in Q areas.

The stop work order is discussed below in more detail.

BWST:

All outstanding NCR's have been closed. However, construction work is constrained until QC inspectors have completed training.

Yard Area:

The investigation of the soils in the area of observation well OBS 4 is continuing, but has been temporarily halted because of the stop work order on drilling.

Assessment Team Observation - Construction Consumers Power and the Contractor have recognized a need to facilitate and improve com=unications between the various organizations which participate in the underpinning and remedial soils work.

To this end, a human relations oriented seminar centered around " Team Building " and a supervisory level weekly meeting have recently been initiated.

The participants in the seminar ranged from.anagement through supervisory level.

The purpose of the " Team Building " seminhr is to develop common understand-ings of the goals of the various organizations involved in the underpinning and remedial soils work.

The weekly meeting for the supervisory level personnel is an outgrowth of the " Team Building " seminar.

It provides a forum for the discussion of intsr-organizational problem areas and for development of solutions of managerial problems.

Although, on their own, these meetings will not necessarily have a direct impact on the construction nor the quality of the work, the Assessment Team believes that improved communications will reduce the impacts of unforseen problems which have hampered work progress.

J. O. No. 14358 Midland Plant Units 1 & 2 Independent Assessment of Underpinning As a result of inadvertently drillin6 into a portion of the Auxiliary Building foundation wall during the installation of a piezometer, a Stop Work Order on all drilling and pile installation in Q areas was issued.

This problem came about because the excavation permit procedure did not properly emphasize a review of structural drawings.

The portion of the Auxiliary Building penetrated was an exterior beam which is an uncommon structural featuce. The Contractor has taken corrective action by reviewing and revising the permit precedures with respect to the requirements for review of structural details. Previous problems with drilling were assoc-isted with locating undergreand utilities.

In addition, a review of the active excavation permits will be made to assure that an adequate review of structural drawings has been made.

The Contractor has informed the Assessment Team that welding technique sh66ts will be prepared for all welds performed for the underpinning and remedial soils work. This approach will simplify the information required by the welders, supervisory personnel, and MPQAD.

This approach should reduce the

' possibility of error in implementing the work.

The Assessment Team observed the Contractor place plasticized concrete for a temporary slab. Various amounts of plasticizer were introduced to determine the dosage that will eliminate the rapid loss of slump experienced during a previbus placement. In the Team's opinion, test program successfully addressed the concern over plasticizer dosage.

In addition. Team members observed the installation of various soldier beams at the SWPS and concrete placement at pier W 14.

These activities were performed in accordance with the dictates of the project documents.

l Assessment Team Observations - QA/QC The Inspection Reports ( IR's) for the E8 grillage beam installation ( a total of 10 I.R.'s ) were reviewed to determine if activities affecting quality are being sufficiently documented.

The review was conducted by comparing I.R. sign-offs to the corresponding PQCI attribute and the technical requirments. The IR's were reviewed for completeness with respect to inspection documentation, type of inspection, action taken because of any deficiencies, and the identification of inspector (s).

All items were found to be satisfactory.

Nonconformance Identification Reports NIR No.

Description Date (Opened) (Closed) 14 Nut Procurement 8/11/83 15 MPQAD Supervisor 9/19/83 Certification A response to NIR 15 on the certification requirements for supervisors of QC. inspectors was received by the, Assessment Team.

The response was-unsatisfactory and discussed with the QA Section Head. Pending resolution of the NIR, MPQAD has taken appropriate interim action by redirecting technical questions to certified personnel.

Flsher said that FSO has requested interpretatien by Project Engineering of the design intent of certain of the optional load steps.

Present status is rejacking of the grillage at the " lift-off" load which was 130 percent of the specified load. Upward movement of the EPA is at 90 mils. ( CLOSED ITEM )

Item 54-2 :

Pier Kc 10 Lead Transfer Status.

Fisher reported that the status of the load transfer now depends upon the direction given by Project Engineering on the requirements to complete the E8 grillage load transfer. ( CLOSED ITEM )

Item 54-3 : UAT Grouting Status.

Fisher reported that the East UAT grouting is complete and the West should be done today.

The direct-hire group will new return to complete the platform work.

Scott asked how the Contractor intends to change the grout repair procedure over -the turbine mat slab area at pier Kc3.

Fisher said the grout-supplier was on-site and has recommended certain changes be initiated such as improving the adhesion, avoiding early form strippin6 and firming-up the form support.

The Subcontractor will likely install anchors to improve the bonding.

Scott said the modification appeared proper but noted that consideration still should I

be given to insuring that air is allowed to escape during grouting.

Discussion Item 54 E8 Grillage Load Transfer Status.

Rate of pier settlement met criteria, pier locked off. ( CLOSED ITEM )

Item 54 Pier E/W14 Status.

Concrete is scheduled to be placed in Pier W14 today and in Pier E14 tomorrow.

It is planned to place concrete in two separate pours in each pier. ( CLOSED ITEM )

Item 54 Pier Kc-10 Lead Transfer Status.

Lead transfer to pier Kc-10 is scheduled for today. ( CLOSED ITEM )

Item 54 36 Inch Diameter Casing Extraction.

There was a discussion about the loss of ground adjacent to the pit (supported by lagging) at one of the locations of the 36 inch diameter casing.

It was concluded that this loss of ground is not a concern since the area will be excavated and backfilled and compacted in the future. ( CLOSED ITEM )

Item 54 Seal Between SWPS & CWIS.

J.

Fisher indicated that engineering has determined that the seal is ok, allowing dewatering to proceed when necessary. However, the protection for the seal is to be replaced. ( CLOSED ITEM )

Item 54 Reschedule Work on BWST.

The Assessment Team was provided with a revised schedule to complete work at BWST. ( CLOSES ITEM 52-22 )

Item 54 Fly Ash Grout Placement.

John Gaydos advised that instructions for placement of lean flyash, which reference SCP 9.000, Concrete Placement Procedure, will be deleted from SCP 3.000, Excavation and Support of Excavation. Some revisions to SCP 3.000 will also be required to.incluce all the necessary lean flyash placement instructions within this one procedure. ( CLOSES ITEM 53-27 )

In response to a question by P.

Majeski. J.

Fisher indicated that a portion of the embed required dismantling and refabrication in the field in order to permit installation. QC wrote an NCR because a fabrication request was not written. FSO's interpretation of the procedures is that fabrication requests are only required for shop fabrication.

Engineering has been asked to clarify this situation to prevent future reoccurences. All refabrication work was inspected and accepted. ( CLOSED ITEM )

Item 54 Inadvertant Drilling of Auxiliary Building Strongback.

Fisher advised that during the installation of Piezometer BB1, the strong-back on the Auxiliary Building was inadvertantly penetrated with the drill.

Discussion Item 54-19-Drift Between Piers Kc-10 to Kc-9 The drift between piers Kc-10 and Kc-9 has been started. ( CLOSED ITEM )

Item 54 Pier Kc-10 Load Transfer.

Pier Kc-10 was loaded yesterday.

The proof load test was met and the load is being held constant on active jacks to meet the rate criteria. ( CLOSED ITEM )

Item 54 BWST Status.

All NCR's written on work associated with the BWST were closed out yesterday and training of personnel is complete. Work has started today. ( CLOSED ITEM )

Item 54 SWPS Lagging Installation Tolerance.

In response to a V. Madill question E.

Cvikl indicated that the bell support drawing will be revised significantly. It is anticipated that grouting will be performed prior to setting lagging and it is likely that allowable tolerances will be increased. ( CLOSES ITEM 53-26 )

Item 54 Reinforcing Detail at Ke Drift Invert.

In response to J.

Springer's question E.

Cviki indicated that the channel section has angles attached to provide horizontal restraint to the support frame. Since the angles are not welded to the frame, the load goes into the diagonal member and not the invert slab which could have otherwise cracked.

Notes of Daily Meeting Independent Assessment of Underpinning Midland Plant Units 1 &2 Consumers Power Company Held at Midland Site Location Midland. Michigan September 29, 1983 Item 54-2b Inadvertent Drilling of Auxiliary Building Strongback.

Because of the inadvertant drilling into the Auxiliary Building strongback' during installation of piezometer BB1, an investigation was undertaken by FSO/MPQAD to determine the cause. It was discovered that the list of applicable structural drawings, used during review pf the excavation permits, was insufficient.

A stop work order was issued ~until a thorough investigation could be made and remedial acticns undertaken.

The excavation permit procedure' is to be evaluated and revised and appropriate retraining accomplished. All active excavation permits will be reviewed to determine if any oversights exist.

This work, and the required approvals, are expected to be completed by October 10. ( OPEN ITEM )

Item 54 Pier E/W 14 Status.

The first concrete pour at Pier E 14 was made on 9/19. However, because of a temperature problem with the concrete, the peur was not completed to the planned construction joint. It is likely that the pier will be completed in two additional pours.

The first to the originally planned elevation of the construction joint.

The second to completion.

The first concrete pour for W 14 will be made today. ( CLOSED ITEM )

Item 54 BWST Status.

Work is restrained at the BWST because MPQAD inspectors have not been certified to perform some inspections.

Sevo is to determine the reason why MPCAD does not yet have certified inspectors for this work, considering that ample time was apparently available during the delays in this area in the past months.

Item 54 Pier KC-3 Load Transfer.

Parish asked if any special measures are being taken because the top portion of Pier KC - 3 is within the fill concrete.

Cvikl responded that the pier is isolated from the fill concrete.

Cviki indicated that Extensometer EX 2 malfunctioned on 9/29 Checks on nearby instrumentation indicated that no significant structural movements were occurring.

The fact that extensometer EX 2 was providing unrealistic data is indicative of the malfunction. ( CLOSED ITEM )

Training CIO attended and evaluated construction training classes conducted on August 29, 30 and 31, 1983. The evaluation of the training was considered to be satisfactory and was in compliance with the requirements of FPG 2.000 " Training ofconstruction Personnel."

Respons'e: CIO received a letter from H.P. Leonard dated August 18, 1983 with three attached memos indicating that MPQAD had completed a review of material-traceability procedures with particular emphasis on. pipe hangers and supports, e

structural materials, piping and weld filler material.

This review was conducted under the direction of M.L. Curland, Principal Quality Advisor for MPQAD. As a result of this investigation, MPQAD has concluded that the systems in use for material identification and control do provide for compliance with ASME code requirements and to CFR 50, Appendix 8 requirements of preventing the use of incorrect material.

This letter with attachments satisfies the documentation request noted in Report No. 6, CIO agrees with the results of the above j

review and the item can be closed.

CIO attended and evaluated a construction training class on September 9,1983.

The evaluation of the presentations of the training session was considered to be satisfactory and was in compliance with FPG 2.000 " Training of Construction Personnel."

Results of the evaluation have been documented on Checklist Number MP-MIS-001E and are available for review.

Computer records for certification of 4 MPQAD personnel trained to meet the requirements of PQCI PW-1.00, Rev. 7 Fabrication / Welding / Heat Treatment and NDE, ASME III Piping were reviewed by CIO.

The requirements of Military Standard 105D " Sampling Procedures and Tables for Inspection by Attributes" as described in the Stone & Webster Quality Assurance Directive (QAD) 7.11 were utilized.

Records of four (4) MPQAD personnel trained to meet the requirements of PQCI PF-1.10 Rev 4 " Pipe Flange Installation and Rework " were 100% reviewed. Computer entries were considered satisfactory.

Zack - HVAC Activities During the period August 15, 1983 through September 12, 1983, CIO performed surveillance inspection of perfomance qualifications of 32 welders.

Bends tests of 13 welders were 100% witnessed by CIO and were detemined to be satisfactory and in compliance with the Zack Welder Qualification Proedure MB-FP-18.

System Ir.teraction Walkdown Walkdowns were performed in the Diesel Generator Room No. 1 Elevation 634' in accordance with Procedure WTP-3-Q, " Procedure for Performing Walkdowns of

,SSIP/S Targets."

The CIO evaluation was performed utilizing attribute Checklist MP-MIS-WTP-3Q.

Seventeen (17) interactions were analyzed and documented, three (3) of which were witnessed by CIO.
