The 24 hour2.777778e-4 days <br />0.00667 hours <br />3.968254e-5 weeks <br />9.132e-6 months <br /> deflection acceptance criteria was satisfied on 9/24 but pier movement shortly thereafter required that rejacking be initiated.
Pier Kc10: No additional work was performed.
Load transfer is delayed pending completion of the E8 load transfer and 3 day rejacking.
Pier Kc9-Kc10 Drift Invert: The invert reinforcing steel was installed in this drift area.
Pier E14:
The pier was excavated and lagged ibll depth to El 565. No Broundwater entered the excavation.
WB Grillage: The load transfer commenced on 9/18.
On 9/23 instrumentation readings indicated the 24-hour deflection acceptance criteria was satisfied allowing the 3-day rejacking period to commence.
Pier Kc3:
The grout patch of the turbine building slab over the pier area was repaired after soundings indicated the presence of a void zone. Sub ~
sequently the grout repair developed cracks so further repair work is required.
Pier Kc2 - Kc3 drift:
The drift was advanced through concrete fill to within a few feet of completion.
Pier W14: The pier was excavated and lag 6ed full depth to El 565. Minor groundwater seepage entered the excavation through the upper fill soils.
UAT: Pressure grouting of the soils adjacent to the west and east UATs continued and is nearing completion. Backfill concrete was encountered in several of the grout hole probes.
SWP3: Exterior well and soldier beam installation continued. The train A service waterline was backfilled allowing access to install several additional soldier beams and dewatering wells.
BWST:
Resolution of all outstanding NCRs has not been completed. No additional
' foundation work was performed.
Yard Area:
Project Engineering initiated an investigation to evaluate the condition of the soils in the vicinity of the partially completed observation well, OBS 4, near the NE corner of the Diesel Generator Building.
Assessment Team Observation - Construction The load transfer of the E/W8 grillage installations was accopolished in accord-ance with the requirements of the project documents.
Throughout the effort, teamwork was apparent between the various supervisors, organizations and craft personnel.
The role of each responsible individual was well defined and well known.
In particular. FSO has continued to take prudent and effective action when upward structural movements met or exceeded the established notification levels. When required, additional data was obtained to supply to Engineering.
EPA upward movement on both the East and West sides has exceded the initial 30 mil notification limit.
Project Engineering has determined that the upward movements observed (60 mils - 90 mils ) 'are not detremental to the EPA structures.

3 J. O. No. 14358 Midland Plant Units 1 & 2 Independent Assessment of Underpinning The FIVP also moved upward durin6 the EPA grillage loading.
The Contractor carefully checked the support system and made any required adjustments to maintain the correct load in the system.
The excavation and support of piers E/W14 progressed according to the design documents. Soils conditions were excellent in both pier locations with only minor groundwater seepage at one of the piers.
Lagging spacing was sufficient to insure good backpacking.
The UAT grouting operation is achieving apparent success. The grout take has been quite high at a number of the holes. The Subcontractor personnel, the Field En61neers and Resident Geotechnical Engineers were knowledgable of the procedures and their respective responsibilities.
After completion of a portion of the effort, the Contractor and Brouting specialists met to review the program and make any adjustments to improve coverage.
Assessment Team Observations - QA/QC 1,
The certification of 4 Q.C. supervisors were reviewed for compliance to project procedures. Three of the supervisors met the procedural certification requirements but one supervisor's certification was jud ed by the Assessment Team to be 6
inadequate resulting in NIR # 15.

A surveillance was made of QA/QC for compliance to the key attributes in the PQCI for load transfer of the W8 grillage. In the Team's opinion, all attributes were properly inspected during the course of the load transfer operation.
3 Observations were made on the Engineering use of the cone penetrometer to evaluate the soil conditions surrounding OBS 4 near the NE corner of the Diesel Generator Building.
In the Team's opinion, the Contractor followed the established site permitting procedures, QA properly verified the equipment calibration records, and the intent of the specification was being met.

Several of the operations were observed that are involvedin soil stabilizing by grouting the zones around the UAT.
The Assessment Team observations indicated the inspection attributes were being properly inspected. In a particular case, the QC inspector displayed a " team attitude." in expediting the opening of an' Inspection Report so that regrouting of certain holes could proceed without delay.

Observations were made by the Assessment Team of the effort underway to review the Quality inspection points used by the MPQAD personnel.
The effort will include an evaluation of the specification and procedure requirements for a particular activity as well as an evaluation of the corresponding PQCI.
The Assessment Team feels the effort, as planned, is an excellent method of evaluating the adequacy of presently defined Quality Inspection Points and identifying needed changes.
