Tec' aical Specification 6.8.1 requires that written procedures be established, implemented and maintained covering the performance of maintenance on safety-related equipment.

Contrary to the above, on June 16, 1983, maintenance personnel performed unauthorized maintenance on the operating Unit 1B centrifugal charging pump, resulting in the pump motor destruction when they failed to abide by the requirements of McGuire Maintenance Management Procedure 1.0 concerning Work Request Requirements.

Response

Duke Power Company agrees that McGuire Nuclear Station Maintenance personnel performed maintenance on the Unit'lB centrifugal charging pump on June 16, 1983 without a work request as required by McGuire Maintenance Management' Procedure 1.0.

This situation was initially reported to the NRC in Reportable Occurrence Report R0-369/83-45. Duke Power Company determined the primary cause of this incident to be due to the lack of a procedure for taking oil samples. The maintenance work involved obtaining an inboard motor bearing oil sample while the motor

was operating. However, due to the lack of a procedure, too much oil was removed from the bearing oil reservoir resulting in damage to the charging pump motor bearings. As a coutributing factor, a maintenance supervisor did not properly follow a maintenance management procedure concerning work requests when he approved the work to be done. The work was performed without a valid work request; however, the event could have occurred even if a work request had been issued since no procedure guidance was available for removing oil from the charging pump motor.

As immediate corrective action, the motor-was repaired and returned to service.

In order to avoid future problems in this area, a procedure was written to document the necessary steps for removin and replacing oil from pumps and motors.

This procedure requires equipment to be.ut of service and red-tagged while work is being done.

It also requires verification of the oil level after completion of maintenance work. Personnel have been reminded that extra precautionary

. measures are expected when dealing with safety related equipment and that Control Room personnel must be consulted when an unusual maintenance situation arises.

McGuire Nuclear Statioa is presently in full compliance with the Technical Spec-ifications.

' 10' CFR 50, Appendix B, Criterion' XVII, as implemented by Duke 1A Topical Report Section 17.2 requires the licensee to maintain sufficient -records on activities affecting quality.

The records shall include data on equipment. qualification, and lue identifiable and retrievable.

Contrary to the above, as of June 1983 the initial calibration recordsffor Unit 2 containment pressure control-system solid strte alarm modules, vere not maintained

. nor retrievable. Calibration data sheets were found to be missing in the system

, turnover package used for operational readiness. This package was completed in January 1982.

. Response:

Duke Power Company agrees'that initial calibration records for McGuire Unit 2 Containment Pressure Control System solid state alarm modules have not been maintained. Discussions with involved personnel indicated that calibration

. data sheets were prepared but are now missing. A search for the missing data sheets has been unsuccessful. Proper. calibration procedures have since been

performed and proper documentation prepared on the alarm modules. Technicians and supervisors have been reminded of the importance of proper operation of this equipment and maintaining applicable data sheets. McGuire Nuclear Station is presently in-full compliance.
