PVNGS UNITS 1, 2 and 3 I.
DESCRIPTION OF DEFICIENCY A study was conducted to identify:all power, control, and instrument penetrations where the power supply has sufficient energy to damage the penetration conductors. The results indicated that for power circuits sufficient primary and backup breakers and/or fuses are provided.
Instrument circuits with a high voltage power supply (approx. 2000 volts) were evaluated as the most likely instrument circuits to require backup fuses.
The evaluation indicated that sufficient energy was not available to damage penetration i
conductors, so all instrument circuits were eliminated from the study. For control penetrations, 106 cases were identified where sufficient energy was available to damage the penentration i
conductors, so backup fuses are required.
II.
ANALYSIS OF SAFETY IMPLICATIONS Given the maximun short circuit current vs. time conditions that-could occur, sufficient energy is available to damage the i
penetration conductors during a single random failure of a circuit overload protection /evice. This could esult in a loss of i
containment integrity. This condition violates RGl.63 and FSAR j
section 1.8 (Confersnce to RG1.63). Accordingly,: this condition is -
j evaluated as repor'.able under 10CFR50.55(e) since if lef t uncorrected it woald represent a significant safety hazard.
III. CORRECTIVE ACTION The condition will be corrected in each unit by issuance of. Design Change Packages ISE-PK-029, 2CE-PK-029, 3CE-PK-029, ISE-PH-036 and -
3CE-PH-036 to provide additional fuses for each of the 106 cases.
With these DCP's implemented,1the design.will provide-aicombination of breakers and fuses or two fuses as primary and. backup overload

protection devices. Where two ' fuses ' are being > used, the primary and '
backup fuses have different ratings and/or model numbersi to. ensure i
that the design is not susceptable toLa common mode-failure, due to-i a manufacturer defect.
