Catawba Nuclear Station, Units 1 and 2 Identification of Deficiency:

Wiring revisions were signed off as complete, when later checks revealed that some problems existed with those panels. This was discovered during routine inspections of later revisions.

Several panels were found to be not in accordance with the revision that was signed of f.

Both Construction and Quality Control inspection had signed off revisions as complete, per CP-147 A complete evaluation revealed that only one of the seventeen items identified by Non-Conforming item 17063 is reportable.

Initial Report:

On September 28, 1983, V. Brownlee and G. Nejfelt, NRC Region 11, Atlanta, Georgia were notified of the deficiency by L. Coggins, S. H. Van Halssen, J. Rowell of Duke Power Company, Charlotte, North Carolina.

Component and/or Supplier:

Fabricated by Duke Power Company, Catawba Nuclear Station.

No component deficiency exists.

Description of Deficiency:

A jumper was installed in 2EATC15 from relay JC (40DX) contact la to relay JA (51VX2) contact la.

A revision to the wire tabulation sheet deleted this jumper. When this revision was completed, the Jumper was inadvertently lef t in the panel. The quality Control inspection of the panel failed to notice that the jumper had not been removed. This connection reduces the two (2) out of three (3) logic of the 51V protective relay scheme to one (1) out of three (3) (less conservative di rection).

Analysis of Safety implications:

The tripping function of these relays is in effect during safety actuation.

Therefore, if the connection between JC (40DX) contact la and JA(51VX2) contact la is left as is, this would result in system operation that is different from that described in FSAR commitments.

Corrective Action:

Specific items addressed in NC1 17063 will be corrected prior to fuel loading.

Training of personnel involved will be conducted to prevent problems of this type from happening again. A complete reinspection of all panels will be conducted prior to final turnover of the system.
