STEAM GENERATOR TUBE LEAKS

Dear Mr. O'Reilly:

l In accordance with Section 6.9.2 of the Technical Specifications for the H. B.

Robinson Steam Electric Plant, Unit 2, the following event is reported.

I' On ' September 5,1983, H. B. Ibbinson Unit 2 was shut down due to a 0.32 GPM l primary to secondary leak in "A" Steam Generator. Upon inspection, it was

. determined that two tubes, Row 14, Column 19 (Hot Ieg) and Raw 28, Column 57 (Gold Ieg) were leaking. . Eddy Current examination of the first tube revealed an indication at approximately 18 inches above the tubesheet. Examination of the L second tube revealed an indication at 23 inches above the tubesheet.

i' A review of the Eddy Current Testing (ECT) tapes from the May,1983 Steam Generator Inspection determined that both tubes exhibited essentially thru-vall indications'and should have been plugged. Based on this information, it was concluded that these indications proceeded to thru-wall as would be predicted by previous calculations.

l: On Sepixuber l'l,.1983, a re-examination of 10% of the Ecr tapes frcm the May, l 1983 inspection of "A" Steam Generator was ocmpleted, and no newly missed l' . pluggable indications were discovered. The two tubes in quescion were plugged and hydrotested satisfactorily.

This event was reported on Septerrber 12, 1963, to the NRC, Region II, and to Mr. G. P..Beatty, Jr., Manager - Robinson Nuclear Project Department.
