Catawba Nuclear Station Significant Deficiency Report No. SD 413/83-07 Final Report of Corrective Action March 3, 1983 l
In response to the referenced reportable item concerning vent / drain i
valve installation and inspection at Catawba Units 1 and 2, the following actions were taken:
1.
Quality Assurance inspectors were retrained to ensure that all inspections performed after September 13, 1983, the da k the problem was recognized, were in accordance with Design Engineering's inter-pretation of drawing CN-1680-47.
2.
All vents / drains inspected prior to September 13, 1983 were reinspected to verify that the intent of design drawing CN-1680-47 was met.
Discrepancies were forwarded to Design Engineering via an attachment to Q-1A 17144 which identified the reportable item.
3.
All discrepancies were evaluated by Design Engineering and approved for incorporation into the appropriate design drawings.
4.
Drawing CN-1680-47 has been revised to more clearly state the criteria for configuration of vents and drains. Prior to distribution, the new revision was reviewed by Quality Assurance, Design Engineering, Stress Analysis and Layout Groups and Catawba Mechanical Technical Support.
Quality Assurance inspectors have been trained to the new revision and training for Construction Technical Support and Crafts was completed on February 22, 1984.
Q-1 A number 17144 reported the generic problem with interpretation of drawing CN-1680-47. The following Q-1As were written to place a priority on Design evaluation of specific vent valve configuration problems identified during reinspection and affecting systems subject to imminent testing or stamping.
17136 17263 17276 17251 17264 17305 17259 17275 The above Q-1As have been cleared.
The reevaluation confirmed that although drawing CN-1680-47 referred to Class A systems, Class A vents and drains were configured on the design drawings and would not have been subject to installation / inspection error due to misinterpretation of CN-1680-47. The new revision eliminated the reference to Class A systems. No additional support requirements were identified as a result of the reinspection performed under Q-1A 17144.
A previous nonconforming item, Q-1A 16510, which was the cause of the initial report of a vent / drain configuration error did require an additional support as a result of reanalysis. __
JP0/HBT/LTP March 5, 1984 In summation, reinspection is complete, all discrepancies have been evaluated, the design drawing CN-1680-47 has been revised for clarification and personnel have been retrained. Action to correct the problem and to prevent its recurrence is complete.
