dated September 21, 1983 In accordance with our referenced letter, this submittal provides the 30-day written report, pursuant to Technical Specification 6.9.1.13.b, for fire-rated assemblies (specifically fire dcors) found inoperable during routine surveillances.

Fire doors found inoperable during the September routine surveillances are identified in the Attachment and include:
20 latching mechanism failures; 27 closure mechanism failures; and 19 doors blocked open typically for construction, startup or maintenance activities.
As each of these deficiencies was identified, compensatory fire watches, pursuant to Technical Specification 3.7.9, Action Statement

were established.
Latching mechanism failures and closure mechanism failuren have been caused partially by a degradation of hardware as a result of high usage and partially by differential air pressure problems.
Plans to upgrade door latches and closure mechanisms into heavier duty hardware on several selected 603rs are being developed.

Mr. J. B. Martin October 18, 1983 Differential air pressure problems are caused by a change in differential air pressure between the Radwaste and Control Buildings.
The supply and exhaust fans to the radwaste building currently do not have an adequate interlock between them to prevent the operation of a supply fan if its corresponding exhaust fan is not operating, and vice versa.
A change in differential pressure has been a recurring problem which necessitates the continual readjustment of fire door closure mechanisms spring tension.
A DCP is being developed to ensure a constant pressure differential across access doors, thereby providing a consistent closure force to reduce the door closure problems.
The administrative deficiencies which have permitted the 19 fire doors to be blocked open are being investigated.
The cause and corrective action will be discussed in the October monthly report.
Results of such surveillances conducted during October 1983 will be reported by November 15, 1983.
