DESCRIFrION OF DEFICIENCY During startup testing activities, the class IE Auxiliary Relay Cabinets furnished by Harlo Corporation were found to have some terminations which did not conform to AMP crimping criteria. The deficiencies consist of the conductor not being visible through the barrel of the termination and the conductor interfering with the termination screw.
Identification of the affected equipment in Unit 1 is as follows:

The attached letter f rom Harlo Corporation provides their acceptance for using Bechtcl's termination criteria per specification 13-EM-306.
At this time, Auxiliary Relay Cabinets for Units 2 & 3 are not inspected. An inspection of relay cabinets in Units 2 & 3 will be made at a future date and NCR's referencing this DER will issued for rework as deemed necessary.
II.
ANALYSIS OF SAFETY IMPLICATIONS This condition is evaluated as reportable. Adjoining connections in a given lug barrel does not adequately assure the degree of electrical continuity required to assure reliable operations.
If inadequate crimped lugs were lef t uncorrected, potential hazards of open circuits could occur which could jeopardize the functional requirements of safety related circuits.
III. CORRECTIVE ACTION All class lE and Non-lE Auxiliary Relay cabinets supplied by Harlo Corportion (Units 1, 2, & 3) shall be inspected for compliance per the following criteria. The terminal lug connection to the conductor is acceptable if the conductor is visible and underneath the crimp connection (i.e.; approximately within 1/16" from the face of the barrel).
In addition, the conductor extending beyond the barrel of the connector does not pose a problem unless it interferes with the termination screw, in which case it should be cut back.
Any connectors which do not meet this requirement shall be removed and have a new AMP connector installed in accordance with Bechtel's termination practices established in construction specification 13-EM-306 (including proper usage and traceability of cramping tools).
f Final Report DER 83-43 Page 2 This condition in Unit 1 will be corrected by dispositioning NCR SE-1574 as " Rework"; The condition in Unit 2 & 3 will be corrected via NCR's to be. issued at a later date. All future NCR's generated as a result of* this problem will reference this DER.
In_ addition to reportability under 10CFR50.55(e), Bechtel PVNGS Project considers the deficiency to be reportable under the requirements of 10CFR Part 21 by the supplier. -Deficiency Evaluation Report 83-43 addresses the reporting requirements specified under 10CFR 21.21(b) (3) with the exception of sub part (vi) which requires the number and location (customers and/or facilities) of other possible defective equipment. A copy of this report is being sent to Harlo Corporation requesting their review for ~ reporting under 10CFR Part 21 including number and location of all components supplied.

Dear Mr. Bingham:
We have reviewed the referenced letter and attachments and as per the telephone notes accompanying your letter, have discussed this matter in several instances with job site personnel. During these conversations and our subsequent investigations, the following points were determined.
The AMP acceptance criteria which is dated June 25, 1981 per your letter was invoked subsequent to the shipment of both Unit I and Unit 2 panels from Hario. tiowever parreirwhitti Wers produced prior to that date were wired in accordance with Bechtel approved Harlo wiring procedure criteria which is essentially the same as the AMP instruction pamphlet. All of these cabinets were inspected by Bechtel SQR personnel prior to shipment and the Harlo criteria was available for use by inspection prior to release.
No comment or question was raised at that time.
In view of your present concerns and to expedite implementation, we will agree to authorize the job site to replace necessary connections using the criteria stated in (NCR) SE-1574 page 3, under the following conditions.
1.
That a detailed list by wire number of each termination that is replaced by Bechtel job site personnel be compiled and supplied to Harlo for record.

That Bechtel/ Client Q.A. surveillance be invoked and a certified Q. A." sign-off be forwarded to Harlo Corporation with the tennination er tabulation stating that the work has been performed in accordance with the established criteria.

That a synopsis of the job site wiring procedures that will be utilized to replace the terminations be furnished to Harlo as part of the field rework documentation package.
4.
That any anticipated cost to Harlo associated' with the replacement of these terminations on the basis of the present reinspection criteria be limited to a maximum of five dollars ($5.00) per termination which approximates the shop cost for performing the task and also limited to the scope presently identified in telephone notes attachment TN-E-3273. Should your further inspection at the resistors significantly increase the quantity you feel requires replacement, the impact will have to be re-evaluated by Harlo.
With regard to Units 2 and 3, we are also agreeable to giving authorization to inspect these units to the same criteria; however the specific authorization to initiate any rework will have to be handled on a unit-by-unit basis as the information is developed and evaluated.
We trust that this meets with your approval. If the foregoing is agreeable to you, you may use this letter as written authorization to proceed in the manner and to extent stated herein.
