Dear Mr. Briggs:
Our September 1,1983 letter transmitted our response to Inspection No. 83-16 conducted on July 12-15, 1983.
Pursuant to your telephone conversation on September 6, 1983, we are providing the requested documentation (Attachments 1 through 7) regarding our response to Item 8 of Inspection No. 83-16.
1 An explanation of the attachments is provided below:
Attachments 1 and 2 -
These EP090-1 forms were prepared, reviewed, initialed and dated in 1981 as shown on the first page. At that time, the reviewer did not initial each individual item. A review performed by Engineering of the subject forms after they were located identified this irregularity.
Subsequently, a second review in July 1983 was performed by another individual who then initialed each individual item.
In addition, although only has two of the four pages of Attachments EP090-1, the first page indicates that a review in 1981 was performed.
Attachments 3 and 4 -
Complete This EP09-1 form does not contain the reviewer's initials and date at the top of the form.
- However, the reviewer's initials are at each individual applicable item with a date at the first item, indicating when the review was performed.

These two EP090-1 forms were prepared in 1981.
The reviewer did not provide his initials at the top of the form, but provided them at each individual applicable item.
However, the date of the review was not included.
Through discussion with the reviewer, it has been determined that the review did occur in 1981.
A general revision to Engineering Procedure EP 090 Control of Design and Design Verification was performed in 1982.
Presently, Attachment EP 090-1 explicitly requires the reviewer's initials and date at each individual item.
