Pursuant to the requirements of Appendix B, Part II, Nonradiological, Environmental Protection Plan, Section 3.2, a copy of the report to the Michigan Water Resources Commission notifying them of a failure to collect a 24-hour composite sanple on September 17, 1983, is attached.
This report is required by Amendment 54 to Facility Operating License No. DPR-58 and Amendment No. 40 to Facility Operating License No. DPR-74 dated May 6, 1982.

Dear Mr. Marks:
On Seotember 17, 1983, when collecting the daily sample from the turbine room sump 24-hour composite sampler, the sampler was found to be empty. A grab sample of the sump effluent was made for reporting purposes, investigation revealed a stuck valve.
The pump valve was repaired and the compositor was returned to service.
