A.2.g Conditions Prior to Occurrence Unit 1 - Refueling Outage Unit 2 - 680 MWe Unit 3 - Cold Shutdown Identification and Descriotion of Occurrence Performance of Surveillance Instruction 4.7.A.2.g-3 revealed that the leakage rates for each "A",
"C",
and "D" line MSIV's were a total of 2615 5 SCFH, 37.6 SCFH, and 2652 7 SCFH, respectively. Using conservative assumptions, the leak rate applicable to each valve is 1307.8 SCFH for "A" line MSIV's,18.8 SCFH for "C" line MSIV's, and 1326.4 SCFH for "D" line MSIV's.
This causes the total type B and C tests to exceed 60% La or 655 9 SCFH (Technical Specification 4.7. A.2.g) for "A" and "D" line HSIV's and causes the technical specification limit of 11.5 SCFH (Technical Specification 4.7. A.2.1) to be exceeded on these three MSIV lines.
Aeoarent Cause of Occurrence :
Leakage past the main valve seat on the "A" and "D" line MSIV's and packing and seat leakage on "C" line MSIV's.
