dated March 29, 1982 In accordance with our commitments in Reference (1), this letter represents the 30-day written report, pursuant to Technical Specification 6.9.1.13.b, for fire-rated assemblies (specifically fire doors) found inoperable during routine surveillances.

Due to the large number of fire doors found inoperable during routine i

daily, weekly, and monthly surveillances of fire doors, we believe grouping of all such reportable occurrences into a monthly summary report, rather than multiple reports during the month, is appropriate.

This approach was previously utilized in the case of multiple reports required for inoperable fire-rated assemblies l

(specifically fire barrier penetrations) for which monthly summary reports were submitted beginning with Reference (2).

Our conclusion that monthly summary reporting is appropriate is supported by 10 CFR 50.73 which, when effective, will eliminate the requirement for such reports.

Fire doors found inopetable during routine surveillance can be grouped into three categories:

latching mechanism failures; closure mechanism failures; and doors blocked open typically for construction, startup or maintenance activities.

As these deficiencies are identified, compensatory fire watches, pursuant to Technical Specification 3.7.9 Action Statement

We have completed a review (limited to fire doors now inoperable) and verified that there are no inoperable fire doors for which compensatory measures have not been implemented.

flowever, exanination of prior surveillance records has not been undertaken since it would not contribute to the identification or implementation of required corrective actions.

As corrective action, we have revised the Fire Protection and Maintenance Surveillance procedures and checklists to ensure that proper compensatory measures are initiated for any fire protection equipment found inoperable during routine surveillances and that surveillance results are promptly evaluated for reportability.

In addition, we have conducted special training of personnel responsible for the surveillances to assure understanding of surveillance and Technical Specification requirements.

We are reviewing methods of preventing closure mechanism and latching mechanism failures and will report the proposed corrective action in the next report.

Results of fire door surveillances conducted during August 1983 are presented in the attached.

Results of such surveillances conducted during September 1983 will be reported by October 18, 1983.
