' By letter dated October 6,1994, the NRC issued a Notice of Violation (NOV) as described in' Inspection Report 50-14, 414/94-24. The NOV cover letter also suggested that increased management attention was warranted in the area of equipment failure troubleshooting / root cause determination.

By letter dated November 7,1901, Duke responded to this NOV, indicating that certain corrective actions would be taken. These corrective actions constituted commitments. Our response stated that a consistent root cause process had l

been drafted. At that time, it was anticipated that this process would be in use i

on a trial basis by December,1994, and that full implementation into the work management process would occur by early 1995.; Additionally, we indicated an enhanced maintenance troubleshooting procedure.should be implemented by i

December 31,1994, and would be integrated into the overall root cause process.

1 Upon further evaluation, a managment decision was made to broaden the scope of our corrective actions to include developing a common troubleshooting procedure for use at all Duke nuclear sites. The development
