Dear Sir:

The NRC was notified of a potentially reportable deficiency in the l

referenced telephone conversation. At that time, it was estimated that a determination of reportability would be made within thirty (30) days.

l Due to the extensive investigation and evaluation required, an Interim Report is attached.

It is now expected that this information will be finalized by November 30, 1983, at which time a complete report will be submitted.

INTERIM REPORT - DER 83-53 POTENTIAL REPORTABLE DEFICIENCY ARIZONA PUBLIC SERVICE COMPANY (APS)

PVNGS UNIT 3 I.

Potential Problem Construction installation of Type N embed plates in Containment 3 failed to meet specification and WPP/QCI requirements.

A damaged beam in NW Qual at the 100' 6" elevation was being removed per NCR CC-4043 disposition, when it was discovered that (2) Type N insert plates were installed without the (4) 1" x 9" Ig. anchor bolts. Concrete post placement inspection of insert plates for 3C081 has not yet been performed.

In addition the following listed NCR's summarize the embed plate / anchor bolt problems by specific NCR number.

Approach To and Status of Proposed Resolution Bechtel Engineering is reviewing the respective NCR's to provide reportability disposition and technical justification for corrective action.

III.

Projected C.ompletion of Corrective Action.and Submittal of the. Final Report Evaluation of this condition and submittal of the Final Report is Forecast to be completed by November 30, 1983.
