Dear Sir:
The NRC was notified of a potentially reportable deficiency in the referenced telephone conversation. At that time, it was estimated that a determination of reportability would be made within thirty (30) days.
Due to the extensive investigation and evaluation required, an Interim Report is attached. It is now expected that this information will be finalized by April 10, 1984, at which time a complete report will be submitted.

h INTERIM REPORT - DER 83-87 POTENTIAL REPORTABLE DEFICIENCY ARIZONA PUBLIC SERVICE COMPANY (APS)
PVNGS UNIT 1 I.
POTENTIAL PROBLDI While attempting to close valve iPS1AV470, it was discovered that the handwheel replacement adaptor nut was missing. Valve IPS1AV47C could not be closed. The valve is supplied by Borg-Warr.er.
The investigation of the failure of IPSIAV470 revealed that the rising stem was longer than the operator bridle. This did not allow the valve 1PSlAV470 to open fully.
The valve was found to be 1-1/2" from full open.
II.
APPROACH TO AND STATUS OF PROPOSED RESOLUTION Bechtel engineering is corresponding with Roto Hammer (the remote operator vendor) to advise them of the condition and to ensure that the correct assemblies are provided for Units 2 and 3.
Bechtel construction will revise the installation procedure (special CIP 521.0) to require documented verification that the bridle being installed, is the size specirled for the particular valve for all future installations. Unit 2 will be required to inspect bridles installed prior to issuance of the procedure change. Unit 3 has not yet installed any of these assemblies. Additionally, the Construction Inspection Procedure will be revised to clarify the method to ensure that the position indication is proper.
Also, engineering is preparing a walkdown package to reinspect all safety-related valves in Unit 1 utilizing Roto Hammer remote operators. Any nonconforming conditions will be documented and included in the final report to DER 83-87.
III. PROJECTED COMPLETION OF CORRECTIVE ACTION AND SUBMITTAL OF THE FINAL REPORT Evaluation of this condition and submittal of the Final Report is Forecast to be completed by April 10, 1984.
