Dear Sir:

The NRC was notified of a Potentially Reportable Deficiency in the referenced telephone conversation. At that time, it was estimated that a determination of reportability would be made within thirty (30) days.

Due to the extensive investigation and evaluation required, an interim Report is attached. It is now expected that this information will be finalized by September 29, 1983, at which time a complete report will i

I.

Potential Problem During hydrostatic testing, Borg-Warner Valve 2JCHAHV524 was i

manually closed to the' extent that the 5/16" bolts mounting the actuator to the yoke failed.

1 The valve cannot hold pressure, it leaks past the seat.

Damage to the valve-stem can occur if greater than thirty (30) i pounds of rim pull is applied to the handwheel, as reported by Combustion Engineering in Letter V-CE-20369.

II.

Approach To and Status Of Proposed Resolution CE's initial investigation indicates that the manual handwheel rim pull, specified as forty (40) pounds, is in excess of valve.

design. CE has advised APS to limit manual operation of Valves CH-255 and CH-524 for all three units to twenty-five (25) pounds or less as an interim solution. A long-term solution to this problem is being investigated by Borg-Warner and CE.

Bechtel Engineering is corresponding with CE to provide reportability disposition and technical justification for corrective action.

III. Projected Completion of Corrective' Action and Submittal of'the Final Report Evaluation of this condition and submittal of the Final Report is forecast to be completed by September 29, 1983.
