Dear Mr. Willis:

I confirm that I have agreed with Carolina Power & Light (CP&L) Company that I will support the Shearon Harris Nuclear Power Plant (SHSPP), at the request of the Plant General Manager, Shif t Foreman, or designated alternates, and provide medical supervision and care for contaminated and injured CP&L employees and other personnel from the plant at Rex Hospital or at SHNPP.

I am familiar with the CP&L SHNPP Emergency Plan and the relevant emer.;ency procedures. I understand that injuries could involve ionizing radiation.

I will participate in establishing procedures for receiving radioactively contaminated patients and will participate, whenever possible, in annual plant medical emergency drills involving a simulated contaminated individual.

I will provide medical supervision and treatment within the limitations of my training which includes courses in handling radiation accidents (Note: CP&L will sponsor this training, including tuition, salaries, and expenses). I will respond promptly to a request for assistance by the Plant General Manager, the Shift Foreman, or designated alternates. I will make such a request my first priority unless to do so would, in my professional judgement, create an unac-ceptable risk of physical harm to a patient whom I am treating at the time the reques t is made , in which case, I will advise the above named individuals immediately of my inability to respond to his request and will make every effort to minimize the duration of my unavailability.
