NRC Letter dated June 16, 1983
Dear Mr. Collins:

This letter is in response to the Notice of Violation received as a result of inspections ccnducted at Fort St. Vrain during the per.iod May 16-20, 1983.
The following response to the items contained in the Notice of Violation is hereby submitted:
1.
Failure to Follow Procedure Technical Specification 7.4.a,
" Procedures, Administrative Controls," states, in part, that, written procedures shall be' established, implemented, and maintained Licensee Procedure SR-5.4.5-M, "PCRV Cooling Water Flow Scan Functional Test," Issue 16, effective date March 3, 1983, states in Step 5.2.1, " Place flow scanner print controller on I-131 to PRINT and obtain a scan of all thirty-six (36) flow channels.

Contrary to~ the above, Test Procedure SR-5.4.5-M was conducted on April 15, 1983, and April 22, 1983, without recording the dat'a specified in Step 5.2.1.
This is a

The corrective steps which have been taken and the results achieved:
The purpose of the surveillance is to show that the PCRV cooling system flow scanner readout is operable and that the alarms are functionally checked.
The alarms were i
checked with the surveillances in question and the flow scanner readout for the weeks involved can be shown operable from data taken on PM0/R-26-BiM.

Corrective steps which will be taken to avoid further violations:
Table I of SR 5.4.5-M has been revised by adding a column for recording the flow values taken from the flow scanner.
In addition the personnel involved in the tests were s
' reminded to follow the procedure and to initiate Procedure Deviation Reports per the Administrative Procedures as necessary to make the test correct.
A memo will also be sent to all Results personnel to restate this.

Fort St. Vrain Procedure Q-13, Issue 4, " Handling, Storage, and Shipping," Step 4.1.c,
- states, in part,
- that,
" Storekeeper responsibilities to protect items in his custody in accordance with requirements specified by FSV-STD-2...."
o Whereas FSV-STD-2, Issue C, " Receiving, Storage, Handling, and Cleanness During Wanehousing Activities at Fort St.
Vrain,"
Step 4.2.d, states, in part, that, ".
. temperature should be measured at existing thermostats and at locations where special temperature considerations are applicable to an item, and. logged as shown.in Attachment 7.4.
Readings should be taken each regular workday in early morning and mid-aftarnoon."

. Contrary to the above, the licensee failed to maintain a temperature log in the main warehouse, nor was any other reliable form of temperature monitoring and recording being used at the time of the inspection.
This is a

The corrective steps which have been taken and the results achieved:
The responsibility for recording temperatures in the warehouses has been assigned to the Senior Storekeeper.
(2)
Corrective steps which will be taken to avoid further violations:
This activity will be supervised by the Scheduling / Stores Coordinator and the log sheets will be returned to the Administrative Shift Supervisor at the end of each month for review.
(3)
The date when full compliance will be achieved:
August 1, 1983.
