In accordance with our referenced letter, this submittal provides the 30-day written report, pursuant to Technical Specification
.3 6.9.1.13.b, for fire-rated assemblies (specifically fire doors)

found inoperable during routine surveillances.
In addition, in the referenced report for November 1983, the number of doors
-E reported inoperable due to latching mechanism failures was incorrect.
Enclosed please find a corrected attachment.
7 Fire doors found inoperable during the December routine surveillances are identified in the Attachment and include:
-4 25 latching mechanism failures; 16 closu:e mechanicm failures; and 25 doors blocked open typically for construction, startup or maintenance activities.
As each of these deficiencies was identified, compensatory fire watches, pursuant to Technical Specification 3.7.9, Action Statement
'a', were established.
i As reported previously, Design Change Packages (DCP's) are being implamented to upgrade door latches and closure mechanisms into heavier duty hardware and to ensure a constant pressure e
differential across the access doors in the Radwaste and Control Buildings in order to reduce the door closure probicra.

4 as these DCP's are implemented.
As additional instances of door mechanical problems are identified which are not being dealt with by the DCPS's addressed herein, we are documenting these on Nonconformance Reports (NCR's) to assure systematic technical

Mr. J. B. Mortin January 18, 1984 Since implementation of the Fire Barrier Removal and Reinstallation Request (FBRRR) Procedare, effective December 8, 1983, the number of fire doors incorrectly blocked open has decreased, even with increased traffic due to the recent Unit 2 outage.
Additional training on implementation of the FBRRR Procedure will be provided to appropriate Station personnel.
As long term corrective action, the installation of utility penetrations adjacent to the doors for the routing of temporary cables, conduits and/or hoses to avoid blocking open the fire doors is under consideration.
This monthly follow-up report and those sent previously provide updated information associated with an ongoing condition initially reported in 1983.
Accordingly, we will continue these follow-up reports identifying door status and corrective actions in lieu of providing notification and reports associated with the revised 10 CFR 50.72 and-50.73.
The report covering inoperable doors
