FINAL REPORT - DER 83-1 DEFICIENCY EVALUATION 50.55(e) i ARIZONA PUBLIC SERVICE COMPANY (APS)

PVNGS UNIT 1 & 2 I.

DESCRIPTION OF DEFICIENCY A 10CFR Part 21 report was issued by Parsons Peebles-Electric Producte, Inc., pertaining to improper application of a Vestinghouse Type ECT current transformer in the paralleling circuit of emergency diesel generator static exciter voltage regulator subsystems. These transformers were identified to exist in Palo Verde control panels by supplier on June 17, 1982, and Bechtel PVNGS Project was notified on June 18, 1982, via Telex No. 168761 (attached). Bechtel Problem Investigation Request LA-82-59 was subsequently filed requesting the condition to be reviewed.

II.

ANALYSIS OF SAFETY IMPLICATIONS This condition is evaluated as reportable. The subject trans-formers may not be capable of performing their safety related function due to temperature rating problems.

III. CORRECTIVE ACTION Corrective action has been taker in the form of the issuance of l

Design Change Package 1SM-DG-016 for Unit 1 and Design Change l

Package 2CU-DG-016 for Unit 2.

The Unit 3 equipment will be l

corrected by the supplier prior to delivery to the jobsite.
