Dear Mr. Haynes:

NO. 1 UNIT ACCUMULATORS TAGGING ERROR, FEBRUARY 10, 1983 The purpose of this letter is to describe a valve safety tagging error discovered on No. 1 Unit and action to be taken to preclude future occurrences of this type.

On February 10, 1983, an operator was dispatched to remove the Shift Supervisor's tags from the circuit breakers frr No. 1 Unit Accumulator Motor Operated Valves. The valves were to be repositioned at that time as directed by the procedure for the reactor coolant system heatup which was in prcgress. The operator discovered six additional tags on each breaker which should have been on No. 2 Unit Accumulator Valves. Upon discovery, all other associated tags were ver.fied to be properly placed. The tagging error posed no significant threat to personnel, equipment or the health and safety of the general public as the valves were in their proper position for the operating mode.

In addition to the existing practice of performing a surveillance on selected safety related valves and circuit breakers on a daily basis, an additional verification of twenty-five percent of the safety significant tagging will be performed. This additional verification will be performed by a supervisor. The tagging applications to be verified will be designated by the Senior Shift Supervisor and documented on a form developed for this purpose.

To Mr. R. C. Haynes 2/15/83 As a result of this incident and other previous occurrences of a similar nature, Public Service Electric and Gas Company will perform a human factors review of the station with the objective of making recommendations to prevent recurrence of this type of incident. The recommendations will be evaluated and implemented as appropriate.

The increased verification described above will be continued until such time as the human factors review is completed and the appropriate recommendations incorporated and reviewed with your office. Should you desire additional information, please do not hesitate to contact us.
