l DOCKET No. 50-266 UNIT HAME point Beach Unit i DATE June 6, 1991 i COMPLETED BY D. C. Peterson TELEPHONE 414/755-2321, Ext. 361 Unit i returned to service May 21 following a 47-day refueling and maintenance outage. A reactor trip from power occurred at 1146 on May 30. The automatic scram was caused by deenergization of the rod reactor protection instrument bus due to invertor failure. The unit was returned to power at 2337 the same day.
During this period, Unit i experienced an event which is reportable in accordance with 10CFR50.73. On May 10, during steam generator crevice flushing, the primary system temperature exceeded 200'r without establishing containment integrity.
Immediate corrective action consisted of cooling down the primary to less than 200'F. This event is detailed in Licensee Event Report 91-004.
The Unit i service Water piping supply and roturn for the containment Accident Fan Coolers was chemically oleanad; a 36 percent improvement in flow was noted.
Safety-related maintenance includodt internal cleaning of service water check valve to auxiliary food pump cooling SW-135A, '
roworked internals of auxiliary fced to steam gonorator check valve AF-101, calibrated and adjusted position indicator on control board 1C03 to match local indication for auxiliary food dischargo valvo AF-4000, reworked and internally cleaned regenerative heat exchanger inlot check valve CV-370, ropacked steam gonorator blowdown isolation valve MS-5959, environmentally qualified motor to load splices and lug connection for containment spray pumps F-14A and B, installed new packing and stem on steam throttis valve to the turbino auxiliary feed pump TV-2002, inspected and cleaned pressure switch on steam dump arming control PS-2142, residual heat removal inlet check valvos, heat exchanger inlet arid outlet valvos, heat exchanger cross connects, and inlot and outlet flow control valvos were cleaned and re-torqued, repairs to pressurizer pressure control valve RC-431A.
