MONTHLY OPERATING REPORT LICENSEE EVENT RF10RT i

l The following Licensee Event Report was submitted during the month of November,1994:

LER 94-019 On October 19, 1994, a Station Blackout (SBO) power supply failed to start during a functional test. Subsequent investigation determined a control power modification made in May,1994 without GPUN review prevented the unit from starting in a loss of power mode.

Interniption of AC power to a trip circuit caused a unit trip. An uninterruptible power supply (UPS) was installed to maintain the trip circuit energized. The agreement betwegn GPUN and the operator of the power supply (JCP&L)was revised to include review and testing of unit modifications.
