Letter from Mr. A. B. Beach (USNRC) to Mr. Harold B. Ray (Edison),
i dated September 20,1994 The referenced letter forwarded a Notice of Violation resulting from the NRC Residents' inspection conducted July 1 through August 13,1504, at the San Onofre Nuclear Generating Station, Units 2 and 3. This inspection was documented in NRC Inspection Report Nos. 50-361 and 50-362/94-20, dated September 20,1994.
The violation states, in part,"..on July 7,1994, during performance of Maintenance Order 9402095(sic), Step 6.5.18 of SO23-I-5.6, was performed to install the top half of the sleeve bearing, a step that was not called out in the maintenance order.

l The reason for the violation was inattention to detail on the part of both the individual l
who wrote the maintenance order (MO) 94020952 work plan and the craftsmen l
performing the work. The planner inadvertently specified Step 6.5.19 as the starting i
l point following step 6.5.16 rather than 6.5.18 in the work plan of the MO. While l
performing maintenance on Unit 2 Auxiliary Feedwater Pump 2MP504, the craftsmen l
followed the natural progression of the procedure for reassembling the pump bearings. This included performing Step 6.5.18 which had been inadvertently omitted i
by the Planner as described above. This step is an obviously required step necessary for the work being performed. However, the craftsmen performing the work missed the opportunity to identify that the MO work plan was missing step 6.5.18.

Document Control Desk October 17,1994 Edison initiated a Maintenance Division Experience Report (MDER) to determine causes and establish corrective actions. As a result of that investigation, the iesponsible individuals were counseled on procedural compliance and attention to detail. Additionally, the MDER required the incident to be reviewed with all craft and planning personnel emphasizing the importance of attention to detail and procedural compliance.
As a result of other unrelated events, a series of special day long compliance stand down meetings were conducted by Maintenance Management, during which the need for attention to detail in performing work and procedural compliance was reinforced with all maintenance crews. These meetings, which included the crew involved in this performance weakness, were deemed sufficient corrective action for this event as well.
Maintenance Management has also reinforced the expectation that supervisors are responsible for the performance of their crews as well as their own. In addition, recognizing procedure compliance as a cultural issue, Edison has developed a comprehensive plan to improve procedure compliance in the maintenance area.
Full compliance was achieved on July 9,1994, when we were made aware that the maintenance order did not include Step 6.5.18, and concluded that, notwithstanding the error, the work was in fact performed in accordance with the procedure and in a technically correct manner.
