VIRGINIA ELECTRIC AND POWER COMPANY NORTH ANNA POWER STATION UNITS 1 and 2 FIRE DAMPER SPECIAL REPORT A Special Report dated April 6,1994, Letter No.94-176 Fuel Oil Pump House High Pressure Carbon Dioxide System and Control Room Halon System, informed the NRC of our systematic review of the Fire Protection surveillance requirements. This review involved a line-by-line examination, of the station controlled Technical Requirements Manual, to verify surveillance requirements are completely addressed by station procedures. We further informed the NRC that any similar situations identified during the examination would be submitted upon completion of the review in a single report.

The review of the Fire Protection surveillance requirements is now complete.

On Thursday, September 1,1994, Station Deviation Report (DR) N94-1139 dated August 25,1994, was determined to be reportable by the Station Nuclear Safety and Operating Committee. The subject DR documented the missed surveillance for fire damper inspections required to be conducted every 18 months. The cause of the missed surveillance was due to the fire protection surveillance requirements being originally misinterpreted as not to require a visual inspection of the fire dampers. This misinterpretation was carried forward from the Updated Final Safety Analysis Report to the Technical Requirements Manual. This issue was identified in the Notice of Violation in NRC Inspection Report Nos. 50-338/94-21 and 50-339/94-21. Pursuant to North Anna Technical Requirements Manual specification TR 7.2C and GR 1.0.10, a Special Report is hereby submitted because the surveillance requirement for fire damper visual inspections had not been performed as required.

Requirements for visual inspections have been incorporated into periodic test procedures specific to fire dampers. Subsequently, the fire damper surveillance was satisfactorily completed. No other missed surveillance's were identified following completion of the Fire Protection surveillance requirements review.
