10CFR Part 21 Defect Design of_CRD Housing Band Clamp

Dear Mr. Keppler:

On January 7, 1983, Reactor Controls Incorporated (RCI) informed Illinois Power Company of a potentially reportable defect per 10CFR21 regarding control rod drive (CRD) band clamps supplied by General Electric (GE) for use at Clinton Power S ta tion . Based on an evaluation, Illinois Power is notifying the Commission of a 10CFR Part 21 defect. This report provides information as required by 10CFR Part 21.21(3).

(i) Illinois Power (R. E. Campbell, Director-Quality Systems and Audits) made the initial telephone notification to the NRC, Region III (D. Hays), at 3:45 pm on September 2, 1983, of a 10CFR Part 21 defect. D.

P. Hall, Vice President of Illinois Power Company, by means of this report, hereby provides the written notification to the NRC of the defect.

(ii) The basic component involved is a pipe support (band clamp) for the control rod drive (CRD) insert and withdrawal piping under the reactor pressure vessel and is identified by GE as part number (MPL) B13-D078 P043.

(iii) The band clamps were procured by GE, Wilmington, NC, supplied by GE, San Jose, CA, and installed by Reactor Con trols , Inc. under a GE contract.

(iv) Gulf States Utilities (GSU) audited the GE Wilmington, NC facility to determine if the clamps were fabricated in accordance with the requirements of 10CFR50, Appen-dix B. The audit results contained in the GSU letter to GE (REG-15270, dated June 17, 1983) stated that the band clamps cannot be upgraded for use in a safety-related application because certain criteria of 10CFR50, Appendix B, were not complied with.

Mr. James Keppler Page 2 The band clamps are used for support in a system that is necessary for safe shutdown of the reactor. If any of these supports were to fail, piping stresses could exceed their Code allowable values.

(v) On January 7, 1983, G. Secchi (Reactor Controls Incor-porated) informed P. E. Walberg (Illinois Power Compa-ny) by telephone of a potential defect in CRD housing band clamps supplied by GE for Clinton Power Station (ROC Y-8910, dated January 7, 1983).

(vi) There are one hundred forty-five (145) of the band clamps at Clinton Power Station, one (1) on each CRD housing.

(vii) The GE Master Parts List (MPL) D13-D07 8 identified the insert and withdrawal (I/W) hydraulic lines as Code Classification A (ASME Code,Section III, Class 1, Subsections NA & NB) and referenced a subordinate parts list, 283X769 G001. That parts list included the band clamp as item number 043. The Code Classification column was blank for the band clamp and the I/W hydraulic lines. The MPL and subordinate parts list referenced an installation kit drawing, 768E354, and an outline drawing, 767E724. Both drawings showed the band clamp and included notes of " essential component" and "ASME Code Section III, Class 2." The NSSS contract, Appendix C, Section B, states that GE "will comply with applicable codes, addenda, and Code Cases in effect at the time equipment is ordered".

Based on this information, Illinois Power, Sargent &

Lundy (S&L) , and RCI assumed that these clamps were qualified to Subsection NF. Similar assumptions were made by at least one (1) other project (c.g. Nine Mile Point Unit 2). The band clamps , however, were not qualified to Subsection NF and were intended only as shipping clamps.

The existing clamps will be removed and replaced with a qualified support, meeting the requirements of 10CFR50, Appendix B. Stone and Webster Engineering Corporation is designing the replacement supports, with completion expected by September 19, 1983. The new supports are expected to be procured, fabricated, and installed by February, 1984.

l GE is currently reviewing their design documents in an l attempt to eliminate misinterpretation regarding l qualification of the band clamps.
