Response to Nuclear Regulatory Commission (MRC) Bulletin Number 09-01, Supplement 2, Failure of Vestinghouse Steam Generator Tube Mechanical Plugs.
Gentlemen:
Pursuant to 10 CFR 50.54(f) Toledo Edison provides the following response to NRC Bulletin Number 89-01, Supplement 2. " Failure of Vestinghouse Steam Generator Tube Mechanical Plugs."
The subject supplement requests that actions similar to those requested in NRC Bulletin 89-01, " Failure of Vestinghouse Steam Generator Tube Mechanical Plugs," be extended to include all Vestinghouse mechanical plugs fabricated from thermally treated Inconel-600.
Action Number 1 in NRC Bulletin Number 89-01, Supplement 2, requests that addressees I
verify that information cuntained in Table 2 of Addendum 2 to Revision 3 of Vestinghouse reports VCAP-12244 (proprietary version) and VCAP-12245 (non-proprietary version), " Steam Generator Tube Plug Integrity Sut*. mary Report," May 1991, is correct for plugs fabricated from group 2 heats.
Action 2 of the subject bulletin requests that l
addressees take certain actions concerning Vestinghouse mechanical plugs fabricated from Inconel-600.
Toledo Edison (TE) responded to NRC Bulletin 89-01 b, a letter dated June 19, 1989 (Serial Number 1-886) stating that TE has not procured i
any Vestinghouse mechanical plugs for steam generator tubes at Davis-Besse.
Toledo Edison has taken action to prohibit the use or purchase of Vestinghouse mechanical plugs fabricated fron Inconel-600 at Davis-Besse.

Docket Number 50-346 License Number NPF-3 Serial Number 1-960 Page 2 The NRC has reviewed the information previously provided by Toledo Edison and concludtd in a Safety Evaluation dated June 28, 1991 (Log Number 3502) that Toledo Edison has adequately addressed NPC Bulletin 89-01 and this issue is closed for Davis-Besse.
Toledo Edison notified the NRC by telecon on June 11, 1991 that Davis-Besse utilizes Babcock and Vilcox (B&V) steam generator tube plugs that are fabricated from Inconel-600.
Specifically, tventy-one (21) B&V mechanical plugs fabricated from Inconel-600 vith heats (f material (V945-1 and 2848-2) potentially suset:ptible to primary vater stress corrosion cracking vere identified to be installed in the hot 1 cgs of the steam generators at Davis-Besse.
Toledo Edison informed the NRC in the June 11, 1991 tela:on that 19 of the 21 identified plugs of concern ver* replaced during the sixth refueling outage with NRC approved, (Log Nember 3175 dated February 21, 1990) Alloy 690 material plugs.
The two remaining plugs of susceptible heats vere inspected with satisfactory results during the sixth refueling outage.
The remaining Paa plugs are scheduled to be replaced during the upcoming sever.th refuell,9 outage (scheduled to commenca on August 30, 1991).
Toledo Edison also plans to replace the remaining Babcock and Wilcox Inconel-600 Steam Generator mechanical tube plugs installed in the ev1d legs of the steam generators with Inconel-690 material plugs during the seventh refueling outage.
Since Davis-Besse has no Vestinghouse plugs installed in the Steam Generators; and plans to replace B&V Inconel-600 mechanical plugs, Toledo Edison satisfies the requirements of NRC Bulletin Number 89-01, Supplement 2.
In a meeting held at Davis-Besse on July 30, 1991, Mr. J. B. Hopkins, NRC/NRR DB-1 NRC Senior Project Manager, approved a 34 day extension (August 20, 1991) for Toledo Edison's response to NRC Bulletin 89-01, Supplement 2.
