The NRC was notified of a potentially reportable deficiency in the referenced telephone conversation. At that time, it was estimated that a determination of reportability would be made within thirty (30) days.

Due to the extensive investigation and evaluation required, an Interim Report is attached. It is now expected that this information will be finalized by September 29, 1983, at which time a complete report will be submitted.

INTERIM REPORT - DER 83-36 POIENTIAL REPORTABLE DEFICIENCY ARIZONA PUBLIC SERVICE COMPANY (APS)

PVNGS UNIT 3 1.

Potential Problem During preoperational testing, required for rework generated by FCR 54.747M, the following deficiencies were reported on Unit 3, HVAC System:

1. Control Building, elevation 74'-0", ITT Ceneral Controls Model NH-95 actuators installed on dampers 3M-HJA-M02 and 3M-HJB-M03 exhibited erratic cycling behavior.
2. Oil leakage past internal seals.

II.

Approach To and Status Of Proposed Resolution Preliminary investigation of erratic cycling of actuators has revealed leaking hydraulic oil seals.

Bechtel engineering and Waldinger have contacted ITT General Controls concerning the erratic recycling and the defective oil seal replacement.

III. Projected Completion'of Corrective Action and Submittal of the Final Report Evaluation of this condition and submittal of the Final Report is Forecast to be completed by September 29, 1983.
