Item B addresses a violation identified in Inspectio : Report 50-413, 414/91-11 invo'ving the fai'. ore to realign essential power supplies when removing the IB Diesel Generator from service. This violation was considered an additional example of Violation 50 413/91-09-01 and was not cited.
Violation 50-413/91-11-01 involved a failure to follow pro:edures while restoring the l A Diesel Generator to service resulting in a diesel fuel oil spill of approximately 1500 gallons in the diesel room. A response to Violation 50-413/91-11-01 will be provided by June 20,1991.
