Docket Nos. 50-361 and 50-362 1990 Annual Operating Examinations San Onofre Nuclear Generating Station, Units 2 and 3 The purpose of this letter is to inform you that as a result of being hospitalized for a temporary illness, Arthur H. Triebel was unable to complete an annual operating examination as required by 10 CFR 55.59 during 1990.

1/2 hours of simulator training and on March 27, 1991, Mr. Triebel satisfactorily completed an annual operating examination.

Mr. Triebel last completed an annual operating examination in August 1989. Mr. Triebel is licensed (No. OP-50207-1)-

under Docket Number 55-50147.

l Additionally, in February 1991, during a review of the annual operating examination requirements relating to Mr. Triebel, it was determined that due to a scheduling deficiency, the 1990 operating examinations for Units 2 and 3 operators were not performed until January 1991.

Specifically, during the Summer of 1990, SCE's Nuclear Training Department (NTD) rescheduled Units 2 and 31990 annual operating examinations (which were last performed in August 1989) to January 1991.

At the time of this discovery, the NTD training program inappropriately allowed for a licensed operator who does not complete all of the requirements of the requalification program within the time limits specified by 10 CFR 55.59(a)_to complete them within the first quarter of the following year.

This program feature was intended to provide for completion of individual requirements that had been missed due to illness or other unforeseen events rather than to allow for requirements for a group of people to be deferred.

personnel responsible for scheduling the annual operating exam incorrectly applied this provision to all, rather than individual, operators in order to align the schedule to coincide with the NRC's administered requalificatien examination which was scheduled for completion in January 1991.

Mr. J. B. Martin All Units 2 and 3 Reactor Operators (R0s) and Senior Reactor Operators (SR0s) successfully completed their 1990 annual operating examinations in January 1991 with the exception of; 1) John K. Murphy whose R0 license was terminated on February 15, 1991 for unrelated reasons and 2) Arthur H. Triebel whose circumstances are described above.

The requirements for the annual operating examination have been discussed with appropriate NTD personnel and implementing procedures will be changed to properly reflect the annual operating requirements of 10 CFR 55.59.

In addition, SCE will perform a Quality Assurance evaluation of all other 10 CFR 55.59 training requirements which will be completed by the end of June 1991.

Deficiencies noted during the evaluation will be corrected as necessary.
