Dear Mr. O'Reilly:
On April 13, 1983, analysis of a representative fuel pool ventilation system (VF) carbon filter sample per T.S. 4.9.11.2(b) indicated the sample failed to meet the penetration criteria of methyl iodide of less than 1 percent. The VF system was subsequently declared inoperable per T.S. 3.9.11, and reportable occurrence report R0-369/83-19 detailing this event was submitted on May 12, 1983 at which time it was indicated that a followup report would be submitted when results of a third analysis were received and final corrective actions determined.
The original test results (6.54%) were subsequently proven to be incorrect when two additional laboratory tests, performed by independent vendors, showed the carbon to perform acceptably (0.56% and 0.20%). The VF system was therefore functionally operable, and this incident is not reportable (the reportable occurrence report should be voided). The VF system was reclassified as operable on May 9, 1983, and a new contract order for carbon analysis is being established with a vendor from whom reliable service and results have been received.
