Docket No. STN 50-482 Subj: Potential 10CFR50.55(e) Report - Guyon 1/2" Socket Couplings
Dear Mr. Seidle:
This letter provides an interim report on a potential 10CFR50.55(e) matter concerning linear indications in the base metal of 1/2 inch weld couplings. This matter was initially repor ted by Mr. Otto Maynard of Kansas Gas and Electric Company to Mr. William Johnson of the Nuclear Regulatory Commission, Region IV, on June 17, 1983.
During installation of two 1/2-inch stainless steel couplings, linear indications were discovered visually in the base metal which extended to the toe of the weld. This condition was documented on a nonconformance report and an investigation was initiated to locate any other suspect couplings. The couplings in question are 1/2-inch 3000 pound SA304 stain-less steel with heat code CLU supplied by Guyon.
Investigation has revealed that 50 of the couplings had been purchased and 35 had been issued to the field. The remaining 15 have been placed on hold. Of the 35 issued, 28 have been located and efforts are under-way to locate the remaining 7.
The status of the evaluation and resolution of this concern will be reported to you on or before October 14, 1983.
