Violation 50-369.370/94-15-01 d
Technical Specification 6.8.1, requires that written procedures be established, implemented and maintained covering the activities in the applicable procedures recommended in Appendix A of Regulatory Guide 1.33, Revision 2,1978. Operations Management Procedure 5-3, Technical Specification Action items Logbook, requires that control room personnel review all necessary documents to ensure that all necessary tasks j
are completed before equipment is declared operable. Work order package 94026627, Task 3, required that operations personnel perform a retest of the 282 sump pump following the completion of a periodic i
test.
Contrary to the above, on August 16, 1994, control room personnel failed to verify that operations personnel had performed the required retest of the 2B2 diesel generator sump pump before they declared it operable and removed the 283 sump pump from service, thereby rendering the 2B emergency diesel l
generator inoperable.
This is a Severity Level IV violation (Supplement 1).

Reason for the violation:
b The reason for the violation is inappropria:e Action. The Control Room Senior Reactor Operator j
(CRSRO) declared the 2B2 diesel generator sump pump operable and cleared it from the Technical Specification Action item List (TSAIL) although the pump operability test had not been conducted. Unaware the 2B2 sump pump remained inoperable, Operations personnel on the following shift removed the 283 sump pump from service for maintenance, thereby rendering the 28 diesel generator inoperable. Since Operations personnel were not aware the 2B diesel generator was inoperable, the required offsite power source verification was not performed.

Upon discovery, Operations personnel immediately declared the 28 diesel generator inoperable.

Operations personnel immediately realigned the 2B3 sump pump, restoring the 28 diesel generator to operable status.

Operations personnel immediately performed the verification of offsite power sources in accordance with Tech Spec 3.8.1.1, Action d.

Operations management counseled the involved CRSRO to ensure adherence to written 1

expectations for removing items from the TSAIL.
N'o similar events have occurred since implementation of these corrective actions.
3.
Corrective steos that will be taken to avoid further violations:
Operations management will communicate the details of this event to all licensed personnel who perform CRSRO duties to ensure they are aware of the need to fully verify the operability of equipment prior to removal from the TSAIL. This corrective action will be completed by December 31,1994 4.
Date when full compliance will be achieved:
McGuire Nuclear Station is now in full compliance.
