9 FLORIDA POWER CORPORATION RESPONSE INSPECTION REPORT 83-0/

April 28,1983 VIOLATION Technical Specification 6.8.1.a requires the use of and adherence to ' procedures recommended in Appendix A of Regulatory Guide 1.33, November,1972.

Regulatory Guide 1.33, Appendix A requires procedures that cover procedure adherence and provide instructions for operation of Emergency Core Cooling Systems (ECCS).

Procedure AI-500, Conduct of Operations, Section 4.1 specifies ECCS procedures as procedures that will be followed step-by-step by the person performing the procedure initialing or signing at each step.

Procedure OP-401, Core Flooding System, paragraph 7.2 provides instruction for the pressurizing of the Core Flood Tanks (CFT) and requires a signoff after the completion of each step.

Contrary to the above at approximately 8:44 a.m. on February 25, 1983, personnel were observed pressurizing the "A" CFT without the use of procedure OP-401.

This is a Severity Level V Violation (Supplement 1).

RESPONSE

(1)

FLORIDA POWER CORPORATION'S POSITION: Personnel did pressurize the "A" CFT, without using the required procedure.

(2)

DESIGNATION OF APPARENT CAUSE: This violation was caused by personnel error.

As stated in the Inspection Report, the operators did not realize that a procedure was necessary to perform this routine operation.

(3)

IMMEDIATE CORRECTIVE ACTIONS:

On February 27, 1983, Operations personnel were reinstructed on the importance of using procedures involving the ECCS.

(4)

LONG TERM CORRECTIVE ACTIONS:

The requirements regarding " step by step" signoffs of procedures and the application of that requirement to particular procedures will be reviewed and modified as necessary. This requirement and those procedures identified as requiring step by step signoff will be reviewed in the Operator Recall Program.

(5)

DATE OF FULL COMPLIANCE: Full compliance was achieved on February 27,1983.
