Your Inspection Report No. 50-410/83-01 dated February 28, 1983, identified an apparent violation resulting from an inspection conducted at the Nine Mile Point Unit 2 construction site. Niagara Mohawk's response is enclosed.

o NIAGARA M0 HAWK POWER COP 90 RATION NINE MILE POINT - UNIT 2 DOCKET NO. 50-410 Response to Notice of Violation Attached to NRC Inspection Report No. 50-410/83-01 The apparent violation was identified as follows:

10CFR50, Appendix B, Criterion V states in part that activities affecting quality shall be accomplished in accordance with instructions or procedures.

10CFR50, Appendix B, Criterion II states in part that the Quality Assurance Program shall provide for indoctrination and training of personnel performing activities affecting quality to assure that suitable proficiency is achieved and maintained.

The ITT Grinnell Quality Assurance Manual, QCF-5.4, Control of Production Welds, states in part, "All qualified welders shall have available and be familiar with the required Weld Procedure Specification before performing any welding as required by the Process Planner."

Contrary to the above, welding procedure documents for an ASME Boiler and Pressure Vessel Code,Section III, Class 1 pipe joint and for a Class 3 pipe joint were not made available to an ITT Grinnell welder in the field upon his request as a result of a failure to train craft supervisory personnel in the use and availability of welding procedures.

This is a Severity Level IV Violation (Supplement II).

The following is submitted in response to this item of nonconformance:

The cause of the violation is attributed to ITT Grinnell not providing adequate emphasis during training to foremen, general foremen, and rod issue station personnel as to the importance of the use, location, and availability of welding procedures.

As corrective action to this violation, all foremen and general foremen are being trained and/or retrained as to the importance of making welding procedures available to the welder. All foremen and general foremen are being informed that controlled welding procedures are available at all seven weld rod issue stations. Training for all foremen and general foremen will be completed by April 22, 1983, for both first and second shifts. Training for all weld rod issue station personnel regarding the use and availability of welding procedures will be completed by May 1, 1983.

In addition, welding procedures at each station have been indexed, and the Construction Manager has acknowledged receipt of these procedures.

! To prevent recurrence, ITT Grinnell will conduct training regarding the use and availability of welding procedures for foremen and general foremen on a continuous basis when warranted by welding procedure changes.

In the event that a welding procedure should be revised and/or supplemented, training relative to the new requirements would be provided to the craft and supervisory construction personnel directly affected (i.e., welders, foremen, general. foremen, and rod room attendants).

E Additionally, all craft personnel promoted to supervisory positions will attend a training program on the use, location, and availability of welding procedures.

ITT Grinnell's Construction Superintendent will notify the Training Coordinator when there is a new foreman or general foreman so the required training can be conducted.

ITT Grinnell's Construction Department will notify Quality Control prior to any craft employee becoming a rod room attendant. Quality Control will provide appropriate training on the use, location, and availability of welding procedures before the new rod room attendant issues any welding material.
