' CHANGE DESCRIPTION This Safety Evaluation was written for a new procedure that was for.the overhaul of the Unit 2 Terry Turbine and auxiliary components. This procedure j does not affect the FSAR. l I

SAFETY EVALUATION

SUMMARY

The probability or consequences of an accident ~ described in the FSAR will not be increased or created because the performance of this procedure restores the Auxiliary Feedwater Pump to its original design status.- For the same reason, the possibility of an accident or malfunction of a different type than any previously evaluated in the FSAR is not created. No Technical Specification bases are affected by this procedure. No caanges to the FSAR are required.

-CHANGE TITLE Temporary Heat Trace to 2WTD-H23A,B CHANGE DESCRIPTION

. Heat trace was added to the demineralizer heaters since the heater capacity

-was not enough to maintain alarm setpoint and to prevent tanks from freezing.'

This temporary modification does not affect the FSAR.

SAFETY EVALUATION

SUMMARY

The probability or consequences of an accident described in the FSAR will not be increased or created because the temporary heat tracing is installed to supplement existing heaters to maintain designed temperature parameters. The possibility of an accident or malfunction of a different type than any previously evaluated.in the FSAR is not created because the addition of the low wattage (10 watts /ft) heat tracing will not cause existing temperature limits to be exceeded. No Technical Specification bases are affected by this modification. No changes to the PSAR are required.

Beaver Valloy Powsr Station Unit 2 Addition to-1989 Report of' Facility Changes, Tests, and Experiments Page 3 of-8 GHf 23 TITLE Temporary' Modification'to Limit Switch Wiring on 2CS-PCV456 CHANGE DESCRIPTIQg A closed limit. switch originally used for a computer point was utilized for a red indicLting light on the benchboard in the control room. The wiring wac changed at the limit switch terminal boards. This modification restored benchboard indication to meet Tech. Spec. requirements and does not affect the FSAR.

SAFETY EVALUATION SUMMARI

-This modification removes non-safety related redundant computer point indication so that benchboard indication'can be restored to satisfy Technical Specification requirements. The probability or consequences of an accident  ;

described _in the FSAR will not be increased or created since the computer j indication is not required for any analyzed accident.1 The possibility of an accident or: malfunction of a different type than any previously evaluated in the FSAR is not created because this modification will restore benchboard indication. -No Technical Specification bases are affected by this modification. No changes to the FSAR are required.

CHANGE DESCRIPTION This temporary _ modification returned recorder 2Hvc-FR21' to a 4-20ma DC loop configuration enabling proper indication of fano 2HVC-FN241A & B flow as plant designs intends. l

.l SAFETY EVALUATION

SUMMARY

'l j

This temporary modification provides for proper calibration of the flow recorders; therefore accurate indication is assured during operation. Thus,

the probability or consequences of an accident described in the FSAR will not .

'ba increased or created, For the same reason, the possibility of an accident.

or malfunction of-a different type than any previously evaluated in the FSAR' is not. created. No Technical Specification bases are affected by this modification._ No changes to the FSAR are required.

-QHANGE TITLE 2RCS-LT1320-Temporary Replacement CilANCE DESCRIPTION This temporary modification was to replace a Barton 752 level tranomitter with

-a Rosemount ll54HPSRB transmitter until a Barton transmitter is available.

This change did not affect RVLIS operation.

SAFETY EVALUATION SUHMARY This temporary modification provides temporary replacement components that perform the same function, with the same rollability, as permanent components. Thus, the-probability or consequences of an accident described in the FSAR will not be increased or created. For the same reason, the possibility of an accident or malfunction of a different type than any previously evaluated in the FSAR is not created. No Technical Specification bases are affected by this modification. No changes to the FSAR are required.

nsaver Valley Powar Station Unit 2 Addition to 1989 Report of Facility Changes, Tests, and Experiments Page 6 of 8 CHANGE TITLE 2PHP-21-VPI-LVDT-II - New Procedure GHANGE DESCRIPTION A new procedure was written to establish a safe and correct manner of replacing the LVDTs mounted on the Main Steam Safety Valves used in conjunction with the VPI system. This procedure directly supports previous analysis by ensuring plant equipment operates as described in the .3AR.

SAFETY EVALUATION

SUMMARY

The probability or consequences of an accident described in the FSAR will not be. increased or created since this procedure is used to ensure that installed equipment is maintained and tested to operate as designed and evaluated. For the same reason, the possibility of -an accident or malfunction of a different type than any previously evaluated in the FSAR is not created. No Technical Specification bases are affected.by this procedure. No changes to the FSAR are required.

Beavor Valley Powsr Station Unit 2 Addition to 1989 Report of Facility Changes, Tests, and Experiments Page 7 of 8 CHANGE TITLE Temporary modification for 480V 3 Phase Power Required for Relay Testing j

.gilANCE DESCRIPTION.

480V 3 phase power was required for the rolay calibrations that were scheduled

.for 1R. These relays are located in the Emergency Switchgear (730 el.

Service Building,) and '/ 55 el . Relay Room.. DCP 1045 made this temp mod permanent. Power feed was made from Mcc-2-08 8D and 8F breakers and MCC-2-19-1 2B breaker which are non safety related. A thermal-magnetic molded case circuit breaker was used with protection provided by 30 amp fuses.

SAFETY EVALUATION

SUMMARY

This chango provides temporary non-1E power to the Emergency Switchgear and Relay Room areas. There is no interaction with any system or component important to safety. No unreviewed safety question is involved. No Technical Specification basen are affected by this change. No change-to the FSAR is required.

Bsaver Valley Powar Station Unit 2 Addition to 1989 Report of Facility Changes, Tests, and Experiments Page 8 of 8 CHANGE TITLE EH_74919 - Jumpers for Computer Points Y6035D, Y6036D, Y6037D, Y6038D, Y6599D, Y3608D CHANGE DESCRIPTIOR The Unit 2 SPDS CIA and CIB indications were receiving incorrect information from CPU point Y6035D (2FPW-AOV204), Y6036D (2FPW-AOV205), Y6037D (2FPW-AOV206), Y6038D (2FPW-AOV221), and Y6599D (2HVR-FNA), Y3608D

(?HVR-FNA). The above inputs were jumpered to simulate a signal that will allos the SPDS CIA and CIB indication to actuate properly. This was a IR start-up commitment. DCP 907, TER 902 and SMR 1802 were initiated to permanently correct these signals.

SAFETY EVALUATION

SUMMARY

This change affects only the plant computer system. The modifications prevent incorrect SPDS CIA and CIB indication; it does not affect CIA and CIB operability. There is no affect on any other system or component important to safety. No unreviewed safety question is involved. _No Technical Specification bases are affected by this change. No change to-the-FSAR is required.
